About this file

This document is Exhibit E: Contract Performance Outside the United States or Colorado, a vendor disclosure and notification form required by the Colorado Department of Corrections (CDOC) for non-construction contracts. The form is mandatory for all bidders and must be completed and returned with the bid submission. The solicitation number is DQ 2027000147, and the form applies to purchase order contracts. The document serves as notice to the Department of Personnel when a vendor intends to perform services outside the United States or the State of Colorado, in compliance with Colorado Revised Statutes 24-102-206 and 24-102-205.

Vendors must disclose whether any contract services or subcontract services will be performed outside the United States or Colorado by answering yes or no. If services will be performed outside these jurisdictions, the vendor must specify the type of services (such as software, hosting, technical and product support, server and monitoring systems, or design and development), identify the specific country or state where services will be performed, and provide justification for why it is necessary or advantageous to perform work outside the United States or Colorado. The form requires signature by the contractor's President, Board Chairperson, or Contract's Authorized Signatory, along with printed name, title, and date. The procurement section must verify whether the vendor provided notice within 20 days of contracting or deciding to contract outside the US or Colorado.
