About this file
This is a Certification of Noninvolvement in Prohibited Activities in Russia or Belarus form required by New Jersey state law (N.J.S.A. 52:32-60.1, et seq.) for any vendor seeking to enter into or renew a contract with a New Jersey state agency for the provision of goods or services, or the purchase of bonds or other obligations. The certification requires vendors to confirm their status on the Office of Foreign Assets Control (OFAC) Specially Designated Nationals and Blocked Persons list and to declare whether they have involvement in prohibited activities related to Russia and Belarus. Vendors must select one of three certification options: confirming non-identification on the OFAC list related to Russia and Belarus activity, acknowledging identification on the list without ability to certify compliance, or acknowledging identification on the list while certifying that any Russia and Belarus activity is consistent with federal law, regulation, license, or exemption.
The form applies to all entities defined as vendors under the statute, including natural persons, corporations, partnerships, limited liability entities, governmental entities, and any parent, successor, subsidiary, or entity under common ownership or control. Vendors must provide complete contact information, including name, Federal Employer Identification Number (FEIN), address, phone number, fax number, and email address, along with the signature of an authorized representative and the date of certification. The Department of the Treasury is authorized to impose sanctions, seek compliance, recover damages, declare parties in default, or pursue debarment or suspension for vendors making false certifications in violation of the law.
