About this file
This document is a Scope of Work (SOW) for a contract between the Utah Department of Health and Human Services (DHHS) and a contractor focused on outreach and healthcare access for substance use disorders (SUD). The contract aims to identify and engage individuals aged 12-17 and 18+ who are at risk of or currently experiencing opioid or stimulant use disorders and lack insurance coverage. The primary services include public education, outreach activities, health plan enrollment assistance, and connecting individuals to treatment resources across Box Elder, Cache, and Weber Counties. The contract requires the contractor to conduct monthly train-the-trainer sessions, perform quarterly progress reporting, and achieve specific outcome metrics, including 5% annual increases in outreach activities, enrollments, and related engagement measures.
The contract's financial terms require monthly electronic invoicing through the Utah Department of Administrative Services' Electronic Funds Transfer (EFT) system, with strict documentation requirements for each invoice. Funding is subject to annual allocation, and any unexpended funds will lapse at the end of the funding period. The contractor must have at least three years of Medicaid enrollment assistance experience and two years of experience working with substance use treatment agencies. The contract emphasizes connecting uninsured individuals to healthcare plans, providing federal subsidy information, and facilitating enrollment in qualified health plans, with a particular focus on vulnerable populations including homeless individuals and those involved in the criminal justice system.
