About this file
This is a URS Post-Retired Verification Form used by Jordan School District in Utah to verify whether contractors, owners, partners, or principals have retired from the Utah Retirement Systems (URS). The form requires respondents to disclose retirement status from URS and, if applicable, provide their birthdate and type of business. Contractors and independent contractors (1099 employees) working with the district must complete and submit this form along with their W-9 to the Accounting Department. The form applies to all individuals listed as the contractor, owner, partner, or principal of any contracting business engaging with Jordan District.
URS retirees receiving monthly benefits face restrictions when returning to work with participating URS employers. If a retiree is discovered to have failed to report reemployment to URS on its Post-Retirement Employment form (Form RTRT-27A) in violation of Utah Code Title 49, the retiree becomes liable to repay URS any overpayment resulting from the unreported employment. Retirees are directed to contact URS's Retirement Benefits Department at 801-366-7770 before beginning work with Jordan District to complete and file the required Post-Retirement Employment form.
