About this file
This is a URS Post-Retired Verification Form used by Jordan School District in Utah to verify whether contractors, owners, partners, or principals have retired from the Utah Retirement Systems (URS). The form requires respondents to disclose their retirement status, and if applicable, provide their birthdate and type of business. The form must be submitted to the Accounting Department along with a W-9. This verification applies to independent contractors paid through Accounting (Form 1099) as well as employees paid through Payroll who work for Jordan School District or other participating URS employers.
URS retirees receiving monthly benefits face restrictions on re-employment with participating URS employers, whether as employees or independent contractors. Retirees are required to complete and file Form RTRT-27A (Post-Retirement Employment form) with URS before beginning work with Jordan School District by contacting URS's Retirement Benefits Department at 801-366-7770. Failure to report re-employment violates Utah Code Section 49-11-1207(2) and subjects retirees to liability for repaying URS any overpayment amounts resulting from non-compliance. This verification requirement applies to all contractor agreements with Jordan School District, including the current Windows Device Management Software - Ivanti contract opportunity.

If it is discovered that a URS retiree failed to report reemployment to URS on its Post-
Retirement Employment form (Form RTRT-27A) in violation of Title 49, the retiree will be liable to repay URS the amount of any overpayment resulting from the violation to report. See Utah code section 49-11-1207(2).
*The retiree is directed to contact URS's Retirement Benefits Department at 801-
