About this file
This is a Cost Proposal Form (Attachment C) for RFP CS26-18 issued by the State of Utah Division of Purchasing and conducted by the Utah State Tax Commission for the procurement of Point of Interaction (credit card machines) solutions. Vendors are required to submit pricing for six distinct scenarios representing different implementation approaches: purchase of 10 machines, lease of 10 machines, purchase of 100 machines, lease of 100 machines, purchase of 350 machines, and lease of 350 machines. Each scenario requires complete costing for hardware, software, applications, components, system support and maintenance, and project management fees. All cost proposals must reflect the vendor's technical response and specifications, and must include minimum required items such as end-to-end encryption, hardware shipping, deployment/implementation, remote key loading, warranty or repair, help desk/customer support, and NFC payment enablement, as well as optional items including machine stands and cables/wires. The resulting contract will have a term of five years, with vendors required to guarantee pricing for the first year.
Vendors must submit all costs in the designated format with complete transparency; failure to follow the format may result in proposal disqualification. Any yellow-highlighted cells in the cost schedule require vendor input, and any zero entries or blank cells will be assumed by the State to indicate services provided at no cost or built into other line items. Cost evaluation will be scored based on a comparative methodology wherein the vendor with the lowest proposed price receives 100 percent of available cost points (300 total points across all six scenarios, with 50 points per scenario representing 16.67 percent of total cost evaluation). Other vendors receive points proportional to how their pricing compares to the lowest bid using the formula: Cost Points × (Lowest Proposed Price/Proposed Price), except for Scenario 6 (Lease 350 Units), which uses an alternative formula that awards zero points to any proposal more than double the lowest proposed price.

CS26-18 Attachment C - Cost Proposal Form
Points of Interaction (Credit Card Machines)
Vendor Name:
Costing Considerations & Instructions
1. The cost portion of an offeror's proposal will be evaluated based on the Total Proposal Costs for each of the six (6) scenarios provided herein. Offerors are to provide the total cost for implementing and maintaining a Points of Interaction solution per the specifications provided for each of the six (6) scenarios as separate and independent projects. The six (6) scenarios are provided in six separate tabs across the bottom of this spreadsheet.
2. All cells in a yellow highlight throughout this document require a vendor input. If you identify a cost that cannot be properly captured in this Cost Schedule, then provide suggestions for modifying this form to the Q&A board for this RFP.
3. Offeror's response to the specifications (i.e., Minimum Requirements, Technical Requirements, etc.), Offeror's Pricing and Parts List, and this Cost Schedule must be reflective of one another. All costs necessary to implement and maintain the proposed solution under the six (6) scenarios must be provided on this Cost Schedule
4. If zero is entered for any cell or if any yellow highlighted cell is left blank, the State will assume that the related services are either provided at no cost or built into another cost item. If a cost category is not applicable, enter N/A.
5. All scenarios must include at a minimum the cost of the following items: 1) End to End Encryption to payment processing vendor; 2) Shipping of hardware; 3) Deployment/Implementation; 4) Remote Key Loading; 5) Warranty or Repair; 6) Help Desk/Customer Support; 7) NFC Payments Enabled. Other costs must be provided where appropriate.
6. The following optional items should also be included as separate itemized costs: 1) Machine Stands; & 2) Cables/Wires where appropriate.
7. Each scenario is based on a potential request from an Eligible User to the resulting contract. All scenarios are intended to allow offerors to adequately provide a full cost of hardware and software implementation, and to ensure offerors are judged fairly and equally against the other offerors The purchase volume provided in these scenarios are estimates for cost comparison purposes only, and do not constitute any guarantee.
8. Each scenario requires the complete cost to lease or purchase the hardware / software provided by the Offers' Points of Interaction solution.
9. Any deviation from this format may result in disqualification of Offeror's proposal.
