About this file
This is an attachment containing vendor qualification questions issued by the State of Utah Division of Purchasing through the Utah Department of Health and Human Services (DHHS) for solicitation MW26-14, a Request for Statement of Qualifications (RFSQ) to plan, coordinate, and host peer-run conferences for peer support specialists and family peer support specialists. The contract will be awarded to the highest-ranking qualified vendor under Utah Procurement Code 63G-6a Part 15 for design professional services and consultants. The contract term is five (5) years, with the state reserving the right to negotiate final contract terms and compensation with the selected vendor, and to proceed with negotiations with the next highest-ranked vendor if fair and reasonable terms cannot be agreed upon with the first-ranked vendor.
The project budget is $150,000 for the entire 5-year contract term, equating to $30,000 per year, funded through DHHS' Mental Health Block Grant with federal restrictions. The awarded vendor may conduct limited fundraising with DHHS permission to cover expenses ineligible under the federal grant. Vendors must demonstrate at least two years of experience in the mental health or substance abuse recovery community, including work with peer recovery support specialists, and at least two years of experience providing adult education or training. Qualification questions assess vendor experience in conference planning and hosting for 250+ professionals, post-event survey administration, pre-event marketing, data management, staffing coordination, planning committee establishment, event content compliance, federal funding compliance, and budgetary stewardship. Vendors must commit to providing resumes of 1-3 key personnel and confirm their ability to meet the scope of work objectives within the stated timeframe, with any proposed modifications to project scope or timeline to be outlined for state review.
