About this file
This is a Proposal Evaluation Score Sheet for the Utah Department of Transportation (UDOT) Fiber Optic and ATMS Construction Services Request for Proposals. The document outlines a three-stage evaluation process for contractors seeking to provide fiber optic maintenance, repair, and installation services. UDOT anticipates awarding multiple contracts under this solicitation, which will remain open for the state to accept additional contractor submissions at any time. The contract term extends through March 31, 2028, with no renewal options available. Contractors will be compensated on a line-item basis according to quantities of work performed, and all work must comply with Utah Department of Transportation standards and specifications.
The evaluation process consists of mandatory minimum requirements, technical proposal scoring, and cost proposal evaluation. Stage 1 requires contractors to pass three mandatory criteria: national certification in fiber optic installation, state certification in fiber optic installation, and certification in both Work Zone Safety and SWIP training. Contractors failing any mandatory requirement are eliminated from further consideration. Stage 2 allocates 60 points for technical evaluation across three criteria: demonstrated experience in fiber optic installation (40 points with 8x weight), staffing and equipment resources (10 points with 2x weight), and internal policies, safety standards, and quality control procedures (10 points with 2x weight). Stage 3 allocates 40 points based on cost proposals, with the lowest cost bidder receiving maximum cost points and other bidders receiving points calculated through a ratio formula comparing their cost to the lowest proposed cost. Proposals exceeding double the lowest cost receive no cost points. The total evaluation yields a maximum of 100 points possible.

TOTAL EVALUATION POINTS (100 POINTS POSSIBLE)
Evaluation of Cost Proposals: The offeror with the lowest total cost will receive the maximum available Cost points. All other offerors will receive points as determined by the ratio* of their cost to the lowest offered cost. Final cost scores will be calculated based on the following:
*Ratio Calculation: Points assigned to each offeror’s Cost Proposal will be based on the Lowest Proposed Cost. The offeror with the Lowest Proposed Cost will receive 100% of the cost points. All other offerors will receive a portion of the total cost points based on what percentage higher their proposed cost is than the Lowest Proposed Cost. An Offeror whose proposed cost is more than double (200%) the Lowest Proposed Cost will receive no points. The formula to compute the points is: Cost Points x (2- (Proposed Cost/ Lowest Proposed Cost)).
