About this file

This is a Certificate of Interested Parties and Conflict Questionnaire disclosure requirement document applicable to the City of Lubbock, Texas for the Quincy Park Public Improvement District (PID) Stormwater Drainage Improvement Project. The document outlines Texas Government Code requirements for business entities entering into contracts with local government entities. Specifically, entities must submit an electronic disclosure form to the Texas Ethics Commission before contract execution when the contract requires City Council action or vote, exceeds $1 million in value, or involves services requiring lobbyist registration. The disclosure form must be completed through the Texas Ethics Commission portal, printed, signed by an authorized agent, and submitted to the City of Lubbock. The contract being pursued involves design, landscape architecture, engineering, or architecture and engineering services for stormwater drainage improvements in the southeastern area of Quincy Park near the Quitman Avenue and 91st Street intersection, with an initial term of six months and one optional six-month extension, plus an additional optional scope expansion for construction management and oversight services extending the potential contract duration.

The document additionally requires submission of a Conflict of Interest Questionnaire (CIQ) Form only if the vendor has business relationships that might create conflicts of interest with the City of Lubbock. Per Texas Local Government Code Chapter 176, vendors must file the CIQ with the City Secretary's Office no later than the seventh business day after becoming aware of facts requiring disclosure. Violation of these disclosure requirements constitutes a Class C misdemeanor. The forms are available through the Texas Ethics Commission website, with completed documents submitted to the City Secretary's Office at P.O. Box 2000, Lubbock, Texas 79457, and scanned email copies are acceptable.
