About this file
This is a Mandatory Scored Questionnaire (Exhibit G) for RFQ/P #47-019 issued by Hall County, Georgia, for a complete roof replacement at the Hall County Jail facility located at 2875 Browns Bridge Road, Gainesville, GA 30504. The questionnaire establishes the evaluation criteria for contractor qualifications and proposals, with a maximum total score of 1,000 points allocated using an 80/20 scoring ratio: 800 points (80%) for technical qualifications and 200 points (20%) for cost proposals. Bidders must respond to all ten scored questions to avoid receiving a zero score. The questions assess contractor expertise, workforce composition, project experience, subcontracting arrangements, crew leadership and supervision capabilities, TPO roofing installation experience, daily work schedule and checklist provision, inclement weather procedures and material protection protocols, workforce maintenance and succession planning strategies, and any history of project terminations.
The questionnaire requires bidders to demonstrate at least five sequential years of experience in the services outlined in the Scope of Work and to provide detailed narratives of at least three similar projects completed within the past three years, including project names, entity names, project costs, and reference contact information. Apparent award winners may be invited to participate in interviews, demonstrations, or further negotiations. The document does not specify pricing details, set-asides, incumbent information, funding sources, or contract term and renewal options.
