About this file
This is a reference questionnaire document issued by the Nevada State Purchasing Division, Department of Administration, on behalf of the Nevada Office of the Military, Nevada Department of Adjutant General and National Guard in support of solicitation 43ADG-S3865 for Statewide Fire-Alarm Security Services. The questionnaire is designed to collect client references from current and former clients regarding vendor performance on projects of similar size and scope. Responses are due by June 9, 2026, and should be submitted to the single point of contact, Mason Bondi, at MBondi@admin.nv.gov with the solicitation number included in the subject line. The State requests that all questions be answered, with responses marked as "U/K" (Unknown) or "N/A" (Not Applicable) where appropriate, and allows for additional pages if needed.
The questionnaire employs a rating scale from 0-10 to evaluate vendor performance across multiple dimensions, including knowledge and expertise, flexibility regarding scope and timeline changes, quality of delivered materials and products, staff interaction dynamics, schedule adherence, customer service responsiveness, billing accuracy, problem resolution capabilities, and overall likelihood of future recommendation. Completed questionnaires are designated as confidential business information under NRS 333.020(5)(b) and are not subject to public disclosure pursuant to NRS 333.333. The State reserves the right to contact references directly for further clarification beyond the submitted responses. The questionnaire also requests qualitative feedback on the vendor's most satisfactory service aspects and whether the client would recommend the vendor to their organization again.
