About this file

This file is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form issued by the Incorporated County of Los Alamos Procurement Division in New Mexico for an on-call concrete and paving services contract. The form requires offerors to certify that neither they nor their principals are presently debarred, suspended, proposed for debarment, or excluded from covered transactions by any federal, state, or local agency. Offerors must also affirm that they have not, within the preceding three years, been convicted of fraud, criminal offenses related to public transactions, antitrust violations, embezzlement, theft, forgery, bribery, or other offenses indicating lack of business integrity. Additionally, offerors must confirm they are not presently indicted or criminally charged for such offenses, are not current or former County employees, and are not immediate family members of County employees or public officials. The form further requires certification that offerors have not had any public contracts terminated for cause or default within the three years preceding the certification.

Offerors unable to certify any statement must provide a written explanation with their submission. The form indicates that certification statements will be thoroughly reviewed and may not necessarily preclude consideration for award. Falsification of any statement on the form constitutes grounds for proposal rejection or contract award rescission. The document requires an authorized signer's name, title, date, and signature for submission with the proposal.
