About this file
This is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form required by the Incorporated County of Los Alamos Procurement Division in New Mexico for the On-Call Power Line Contractor Services solicitation. The form serves as a mandatory certification document that offerors and bidders must complete and submit as part of their proposal for on-call power line work, predominantly for make-ready pole changes related to the County's fiber installation project on its electric distribution system. The certification requires authorized signatories to confirm their eligibility to enter into a covered transaction with the County.
The form requires offerors to certify that neither they nor their principals have been debarred, suspended, or excluded from federal, state, or local covered transactions; have not been convicted of fraud, criminal offenses, antitrust violations, embezzlement, theft, bribery, or other crimes affecting business integrity within the preceding three years; have not violated contract performance provisions or ethical standards; are not currently indicted or criminally charged; are not current or former County employees or immediate family members of County employees or officials; and have not had public contracts terminated for cause or default within the preceding three years. Offerors unable to certify any statements must provide written explanations. The form specifies that falsification of any statement constitutes grounds for proposal rejection or contract award rescission, though certification of certain matters may not necessarily preclude consideration for award.
