About this file

This is a proposal amendment for a state contract between the Utah Department of Human Services (DHHS) and the Utah Health Policy Project (UHPP) to serve as community navigators assisting Medicaid members with changes resulting from H.R. 1 policy implementation. The proposal outlines four major strategy components spanning July 15, 2026 through July 14, 2027, with potential continuation beyond this date at DHHS's written request. Strategy 1.A involves maintaining extended Take Care Utah office hours (8 AM to 8 PM Monday through Friday and 10 AM to 6 PM Saturdays) to provide enrollment services to at least 600 individuals, while Strategy 1.B focuses on one-on-one counseling and eligibility screening support for up to 30,000 individuals addressing work and community engagement compliance. Strategy 2.A establishes a Safety Net Transition program for non-citizens losing Medicaid/CHIP coverage by creating educational resources and facilitating transitions to Federally Qualified Health Centers and charity care options. Strategies 3.A, 3.B, and 3.C establish continuous improvement mechanisms including bureaucratic monitoring, bi-weekly Rapid Cycle Review committees, and quarterly impact audits to measure key performance indicators and inform strategy adjustments.

The total proposal budget is $770,000, distributed as follows: Extended Office Hours ($200,000), One-on-One Community Support ($450,000), Safety Net Transition Program ($30,000), Bureaucratic/Systems Monitoring ($50,000), Continuous Improvement Reviews ($25,000), and Quarterly Impact Audits ($15,000). The proposal establishes specific key performance indicators for each strategy, including number of appointments made, individuals assisted, application statuses, enrollment figures, and renewal completions. Expected outcomes include maintaining continuity of coverage by preventing procedural disenrollment, increasing member literacy regarding 80-hour monthly work requirements, and building a self-sustaining community support network. The contract permits budget flexibility, allowing dollars to be shifted between activities and activities to be revised or added within the original budget with written permission from DHHS. Outcome reporting requirements include measuring the percentage of members losing coverage due to procedural reasons versus actual ineligibility and documenting the number of community partners trained.
