About this file

This is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form issued by the Incorporated County of Los Alamos Procurement Division in Los Alamos, New Mexico for the Foundation Piers for White Rock Substation ReBid project. The form serves as a prerequisite certification document that bidders must complete and submit as part of their proposal response. The certification requires the offeror/bidder to attest to their eligibility to conduct business with the county by confirming they are not debarred, suspended, or otherwise ineligible from covered transactions at any federal, state, or local level. The form must be signed by an authorized representative of the bidding company and submitted with the proposal.

The certification covers multiple eligibility criteria including the absence of criminal convictions for fraud, criminal offenses, antitrust violations, embezzlement, theft, forgery, bribery, or falsification of records within the preceding three-year period. Bidders must also certify they have not violated contract provisions, failed to perform satisfactorily on previous contracts, violated ethical standards under the County Procurement Code, are not current or former county employees, are not immediate family members of county employees or public officials, and have not had public contracts terminated for cause or default within three years. The form explicitly states that falsification of any statement constitutes grounds for non-consideration or rescission of a contract award. Bidders unable to certify any statement must provide a written explanation, and all certifications are subject to thorough review by the procurement division.
