About this file
This is a Substance Abuse Prevention Form required by Kane County Forest Preserve District for the Glenwood Park Capital Improvements project, a capital improvement initiative involving site amenities and entry driveway renovation at Glenwood Park Forest Preserve in Illinois. The form serves as acknowledgment and attestation that contractors and subcontractors have established and will implement a substance abuse prevention program for all laborers, mechanics, and workmen employed on the public works project. The document requires the contractor to provide company name, authorized officer signature, officer title, and date of submission, along with an attached copy of their comprehensive substance abuse prevention program.
This requirement is mandated under Illinois State law (Act 095-0635, the Substance Abuse Prevention and Public Works Project Act), which became effective January 1, 2008. The regulation applies to all contractors and subcontractors engaged in public works projects undertaken by the State of Illinois, counties, municipalities, and other political subdivisions. Compliance with this substance abuse prevention program requirement is a condition of participation in the project and must be demonstrated prior to contract award or commencement of work.
