The form mandates that vendors certify compliance with Florida Statute 287.087 by implementing and maintaining a drug-free workplace policy. The certification requires vendors to notify employees that compliance with the drug-free workplace terms is a condition of employment on the project and that employees must report any conviction or guilty plea related to controlled substance violations under Florida Chapter 893 or federal law within five days of conviction. Vendors must also impose sanctions on or require satisfactory participation in drug abuse assistance or rehabilitation programs for any convicted employee. The authorized company representative must sign and date the certification form, affirming that the firm fully complies with all stated requirements and commits to maintaining a drug-free workplace in accordance with Section 287.087.
