About this file

This is an Anti-Human Trafficking Affidavit required by the State of Florida for the Round Island Shelters Columns Replacement project in Vero Beach, Florida. The affidavit serves as a mandatory attestation that must be executed, renewed, or extended for all state and local contracts in compliance with Florida Statute 787.06(13). The project involves replacing thirty deteriorated poured-concrete structural columns across five existing picnic pavilions at Round Island Park, with six columns per pavilion. All work must adhere to approved construction plans, issued permits, Florida Building Code requirements, and applicable federal, state, and local regulations. The contractor must provide all necessary labor, supervision, materials, equipment, temporary supports, disposal, testing, protection, and incidental services to complete the scope of work.

The affidavit requires the undersigned representative to attest under penalty of perjury that the contracting entity does not use coercion for labor or services. Coercion is defined broadly under Florida law to encompass physical force, unlawful restraint or confinement, debt manipulation tied to labor, document confiscation, financial threats, fraud or deceit, and provision of controlled substances for exploitation purposes. The definition of labor encompasses work of economic or financial value, while services include acts performed at another's behest or supervision, including forced marriage, servitude, and organ removal. This affidavit must be signed and dated by an authorized representative of the entity, with printed name, title, and entity name provided.
