About this file
This document consists of instructions for completing Form 1295, the Certificate of Interested Parties, required by Fort Bend Independent School District (FBISD) in Texas for all vendors responding to the Campus Fitness Programs solicitation. All business entities submitting proposals must electronically complete, print, sign, and submit Form 1295 with their bids, regardless of whether interested parties exist. The form requires vendors to identify the business entity filing the form with its location, the governmental entity (FBISD), the project identification number and description of services to be provided, and any interested parties, defined as persons with controlling interests or those actively participating in facilitating or negotiating the contract. Form 1295 must be filed electronically through the Texas Ethics Commission's online filing application at www.ethics.state.tx.us, after which vendors must print the completed form containing a unique certification number, sign it with an authorized agent's handwritten signature, and submit it with their proposal in the Edgar Request Information section in Bonfire.
Compliance with Form 1295 is mandated by House Bill 1295, which amended the Texas Government Code Section 2252.908 and prohibits FBISD from executing any contract resulting from this RFP unless the business entity submits the completed disclosure form. Vendors experiencing problems completing the form should notify the buyer immediately. The example provided shows a completed form for a vendor with an identified interested party, Peter James from Missouri City, Texas, with a controlling interest, executed and certified on June 5, 2022.
