About this file
This is a Form 1295 Acknowledgment of Exemptions document issued by Fort Bend Independent School District (FBISD) in Texas related to a Campus Fitness Programs contract opportunity. The form is required under Texas House Bill 1295, which mandates that FBISD obtain a completed Disclosure of Interested Parties (Form 1295) from any business entity before executing a Board- or Superintendent-approved contract. The document outlines six categories of contracts that are exempt from this requirement, including sponsored research contracts of higher education institutions, interagency contracts of state agencies or higher education institutions, health and human services contracts where value cannot be determined at execution and any qualified vendor is eligible, contracts with publicly traded business entities including wholly owned subsidiaries, contracts with electric utilities as defined by Section 31.002 of the Texas Utilities Code, and contracts with gas utilities as defined by Section 121.001 of the Texas Utilities Code.
Vendors responding to the FBISD Campus Fitness Programs solicitation must comply with the Form 1295 requirement unless their contract qualifies for one of the listed exemptions. If exempt, vendors must check the applicable exemption box, sign the form, and upload it to the Bonfire portal under the HB 1295 Form section in Required Forms. Failure to comply with Texas Government Code 2252.908 may result in contract delays or termination. The vendor certification section requires the authorized representative to attest that all information provided is true and correct and to acknowledge their understanding of compliance obligations.
