About this file

This is a Bid Form Certification document for Upper Leacock Township in Lancaster County, Pennsylvania, related to the Eby & Stumptown Road Bridges Repairs Project (RETTEW Project No. 0573903809). The bidder certification form establishes the terms and conditions under which contractors submit proposals for the bridge repair work. Bidders must acknowledge that they have examined all contract documents, visited the project site, and are familiar with local conditions, applicable federal and state regulations, and subsurface conditions that may affect project costs and performance. Bids must be submitted electronically through the PennBID website, with the bid price entered into the electronic Bid Form available under the "Bid" tab. The contractor must achieve substantial completion by October 15, 2026, and full project completion by October 30, 2026. All bids remain open for acceptance for 60 days following the bid opening date, and the successful bidder must sign and deliver the required contract counterparts, bonds, and supporting documents within 15 days of receiving the Owner's Notice of Award.

Bidders are required to submit several mandatory documents with their bid, including the Bid Form Certification, Bid Bond, Non-Collusion Affidavit, Worker's Compensation Affidavit, Public Works Employment Verification Form, and Non-Discrimination Affidavit, all of which must be uploaded to the PennBID website as a condition of bid submission. The bid form specifies that unit prices must be computed in accordance with Article 13 of the General Conditions, with final payment based on actual quantities rather than guaranteed quantities. The document clarifies that all costs for rock removal during excavation, restoration, testing, and other incidental work not specifically identified in the Bid Form are included in the bid price with no additional compensation available. The Owner reserves the right to select equipment manufacturers and to accept the base bid or include alternate bids when determining the lowest bidder, with bidders required to indicate manufacturer preferences and any associated add or deduct amounts if alternate equipment is selected.
