About this file

This is a price sheet attachment for an Invitation for Bid (IFB) issued by the State of Utah Division of Purchasing on behalf of the Utah Army National Guard for Weather Station Maintenance Services (RR26-34). The procurement seeks a single contract award to provide maintenance and factory-certified calibration services for two remote automated weather stations located at Camp Williams. The contract term is five years, with vendor pricing guaranteed for one year. Offerors must submit costs for all yellow-highlighted line items on the price sheet to be considered responsive; failure to complete all required fields or any alterations to the sheet's formulas will result in bid disqualification.

The price sheet requires offerors to provide an annual total bid price for calibration and maintenance services that is inclusive of all labor, equipment, parts, materials, travel, and fuel outlined in the Scope of Work (Attachment B), excluding separate service call-outs which will be billed individually. The annual maintenance and calibration cost is the evaluated line item, with contract award to the lowest cost responsive and responsible bidder. Additionally, offerors must provide a blended hourly rate for on-call services, including separate rates for standard hours (6:00 A.M. to 6:00 P.M., Monday through Friday), overtime hours (exceeding 40 hours per week), and premium hours (6:00 P.M. to 6:00 A.M. weekdays and all-day weekends and holidays). Offerors must also specify a minimum discount percentage off parts for on-call and overtime services. These hourly rates and parts discount will not be used in bid evaluation but will be incorporated into the future contract if awarded.
