About this file
This Scope of Services document supports a Request for Statements of Qualifications (RFSQ) issued by the State of Utah Division of Purchasing on behalf of the Utah Division of Waste Management and Radiation Control (DWMRC) for laboratory services. The procurement seeks qualified vendors to provide soil and groundwater sample analysis for both radiological and conventional chemical analysis over a five-year contract term. The RFSQ is conducted under Utah Procurement Code 63G-6a Part 15 for Design Professional Service Providers and Consultants, with the highest-ranked vendor selected for contract negotiation at fair and reasonable compensation rates to be determined during negotiations.
The procurement explicitly prohibits vendors from including pricing information in their qualifications submissions. Contract award will be based on ranked qualifications, with the state reserving the right to negotiate with the highest-ranked vendor and, if necessary, proceed to the next-ranked vendor should fair and reasonable compensation, contract requirements, and contract documents not be successfully agreed upon with the initial selection. No specific response deadlines, site visits, or bidders' meetings are detailed in this scope document.

Attachment B: SCOPE OF SERVICES
NC26-81
Laboratory Services for Environmental Samples
PROJECT DESCRIPTION
The scope of work includes all necessary professional services for the following:
General
As part of routine regulatory oversight duties the Utah Division of Waste Management and Radiation Control (DWMRC) collects ground water samples at regulated Low-Level and 11e.(2) radioactive waste disposal facilities as well as active and former uranium mill sites. The samples are collected as part of a regulatory program designed to independently verify laboratory results submitted by the regulated entity. Accordingly, any laboratories currently analyzing environmental samples on behalf of EnergySolutions, Energy Fuels Resources (USA) Inc., Rio Algom Mining LLC, or Anfield Resources Inc. are ineligble to perform the services described herein.
Specifications
The frequency, timing, and actual number of samples collected under this contract is subject to change. The DWMRC is seeking a contract for a period of 5 years during which soil and groundwater samples may be collected and submitted at various times throughout the year, given appropriate pre-planning with the analytical laboratory. Environmental samples (and QC samples as appropriate) will be submitted to the laboratory for both radiological and conventional chemical analysis. The price shall be fixed during the 5-year term.
Attachment 1 is a list of the analytes and preferred method numbers for each of the 4 facilities from which samples will routinely be submitted under this contract. The majority of samples collected will be groundwater, although soil, sludge, and biological samples may occasionally be submitted. Attachment 1 also includes a column listing maximum Reporting Limits for certain parameters based on ground water protection levels (GWPLs) for the sites. If achieving the Reporting Limit for a given analyte is not possible, please note as much in the statement of qualifications package and indicate the Reporting Limit that can be achieved. Likewise, preference will be given to laboratories that can analyze using the listed method numbers, but it is recognized that there will likely not be a laboratory that can accommodate all proposed method numbers.
It should also be noted that ground water samples collected from the EnergySolutions site are highly saline, with total dissolved solids (TDS) values averaging 40,000 to 50,000 mg/L.
Important: For all reported radiological results, the coefficient of variance (CV) must be less than 100% for the data to be accepted by the DWMRC. The CV is calculated bydividing the error term (standard deviation) [s] by the reported value (mean) [x] of the sample result [CV = s/x*100].
All analytical work must be conducted by a Utah-certified laboratory. Accordingly, the successful bidder must be a Utah-certified laboratory or be capable of acquiring the certification prior to conducting services under the contract. The statement of qualification must include a copy of the current primary laboratory certification (Safe Drinking Water Act, RCRA, and Clean Water Act) as well as a specific list of analytes and approved methods by the Utah Bureau of Laboratory Improvement.
SERVICES INCLUDED
The professional services provided by the laboratory for the above described project shall include the following tasks:
Provide DWMRC the appropriate number of sample containers, labels, insulated shipping containers (ice chests), preservatives, insulating foam pads, trip blanks and temperature blanks to perform each sampling episode. Shipping costs to deliver the samples to the laboratory should also be included in the cost. Costs will be negotiated later – do not submit cost for the first phase.
The laboratory shall:
Receive, log-in, and track samples as they are delivered.
| Provide DWMRC within two working days of sample receipt (1) a copy of the completed chain-of-custody record that accompanied the samples during shipment, (2) a completed condition of samples upon laboratory receipt form, and |
| a register form identifying the analyses that are scheduled for the samples. |
Notify DWMRC within 24 hours of sample receipt if problems with the sample quality are identified.
Maintain and document chain-of-custody for each sample as it progresses through the laboratory, including disposal. Chain-of-custody forms will accompany each sample shipment documenting custody from field personnel to laboratory personnel.
Prepare and analyze samples within applicable holding times for the target analytes specified by each referenced analytical method presented in Attachment Should the laboratory be unable to conduct all of the required analytical methods directly, it will be the responsibility of the laboratory to subcontract that portion of the contract to a Utah-certified laboratory that will perform all required analyses in accordance with all contract requirements. [NOTE: Again, it is recognized 100% correlation with the preferred method numbers may not be possible. Please note the method numbers you are able to perform in the statement of qualifications].
Conduct the analyses in accordance with the procedures and protocols specified in the analytical methods referenced in this agreement including matrix spike/matrix spike duplicate (MS/MSD) samples, laboratory control samples (LCSs), surrogate standards, internal standards, laboratory duplicates, and method blanks. The required reporting limits for each analyte shall be those presented in Attachment 1.
The laboratory shall provide the following deliverables to DWMRC within the specified time period.
Laboratory receipt forms as specified under Item 2 within 2 working days of sample receipt.
Laboratory acknowledgment forms within 2 working days of sample receipt to verify the analysis to be performed on each sample.
Analytical results for all samples will be presented to DWMRC in hard copy and electronic disk (tab-delimited file) within 30 days of sample receipt. The analytical report shall be presented on numbered pages with an index or table of contents describing the contents. All analytical results should be reported as detected concentrations or less than the quantitation limit with the appropriate laboratory data qualifiers. All quality control samples shall be clearly linked to the associated samples.
