About this file

This is a Conflict of Interest Questionnaire (CIQ) form issued by the City of Arlington, Texas, Office of Procurement in compliance with Chapter 176 of the Local Government Code. The form is required for any person or entity seeking to contract with the City of Arlington for the sale or purchase of property, goods, or services. The document serves as the standardized disclosure instrument through which proposers must identify any affiliations or business relationships that could create conflicts of interest with the local governmental entity. The form requests the proposer's name, identification of any vendors with business relationships to local government, names of relevant officers, and detailed descriptions of employment or business relationships maintained with corporations or entities in which local government officers serve as officers, directors, or hold ownership interests of one percent or more. Completion and submission of the CIQ is mandatory for all bidders regardless of whether conflicts exist.

If no conflicts of interest are identified, proposers must state the proposer name in section one and indicate "N/A" for all remaining areas on the form; however, a signature is required in section seven in all cases without exception. The CIQ form is available through the Texas Ethics Commission website at https://www.ethics.state.tx.us/forms/conflict/ and must be filed in accordance with applicable state requirements. Updated questionnaires must be submitted in conformance with Chapter 176 of the Local Government Code. The form is included with all submittal documents for the Entertainment District Street Sweeping Services contract solicitation.
