About this file
This is an Insurance Coverage Declaration form for Letcher County, Kentucky, detailing mandatory insurance requirements for contractors bidding on the county's invitation for bid for title services related to the NRCS EWP Home Buyout Program. Contractors must provide proof of commercial general liability insurance with an aggregate limit of $2,000,000 and per-occurrence limit of $1,000,000, comprehensive automobile and water vehicle liability coverage with a minimum combined single limit of $1,000,000 per occurrence for any vehicles used in service performance, and worker's compensation insurance with employer's liability coverage of at least $1,000,000 per occurrence for each accident or illness. All insurance policies, including endorsements, must be attached to the proposal submission, and failure to do so may result in proposal rejection. The worker's compensation policy must include a provision prohibiting cancellation or material changes without thirty days' prior written notice to the county.
The declaration form includes a table requiring contractors to identify their insurance carriers, specify the type of coverage provided, list policy limits in dollars, and provide policy expiration dates for general liability, commercial auto, and worker's compensation coverage. Contractors with owner-operators who have filed a "Notice of Election to be Exempt" from worker's compensation must submit a signed copy of that notice, provided the exemption meets applicable Kentucky Worker's Compensation law requirements. The form establishes baseline insurance protections that all bidders must maintain throughout the contract period to ensure adequate coverage for the title work, closing services, and related activities associated with the home buyout program covering approximately 100 properties.
