About this file

This is an Insurance Coverage Declaration form for Letcher County, Kentucky, related to an invitation for bid for asbestos abatement services at a residential property. The document requires contractors to submit proof of insurance coverage as part of their bid proposal, with failure to provide the required documentation potentially resulting in proposal rejection.

Contractors must provide three types of insurance policies: Commercial General Liability coverage with limits of $2,000,000 aggregate and $1,000,000 per occurrence; Comprehensive Automobile and Water Vehicle Liability with minimum limits of $1,000,000 per occurrence combined single limit for any automotive equipment used in service performance, along with Physical Damage Insurance for owned or rented machinery, tools, equipment, office trailers, and vehicles; and Worker's Compensation with employer's liability coverage of at least $1,000,000 per occurrence for each accident or illness. The Worker's Compensation policy must include a provision stating it cannot be cancelled or materially changed without thirty days' prior written notice to the County. Contractors with owner/operators who have filed a Notice of Election to be Exempt from Worker's Compensation must provide a signed copy of that notice, provided the exemption meets applicable state Worker's Compensation law requirements. Bidders must complete the declaration form and attach all relevant insurance policies and endorsements to their proposal.
