About this file
This Scope of Work document outlines a grant-funded demonstration project administered by the Utah Department of Health and Human Services (DHHS) to link adults with intellectual and developmental disabilities to preventive healthcare and health promotion programs. The project requires the grantee to maintain trained linkage coordinators who will screen at least 50 eligible participants by July 31, 2026, assess their healthcare needs, and refer them to appropriate healthcare providers and evidence-based health promotion programs. Linkage coordinators must conduct follow-up appointments at the three-month mark and perform pre- and post-knowledge assessments to measure program effectiveness. The grantee must comply with DHHS's Data Management Plan, collect and track required data forms including initial participant interviews, screening and enrollment outcomes, and three-month follow-up retention data, and submit quarterly reports by December 5, April 3, June 30, and August 7 of each year documenting successes, barriers, and de-identified participant data.
The total funding is structured across five periods: $110,024.00 from October 5, 2022 to August 31, 2023; $211,255.00 from August 1, 2023 to July 31, 2024; $60,000.00 from August 1, 2024 to July 31, 2025; $37,822.00 from December 1, 2024 to July 31, 2025; and $148,619.00 from August 4, 2025 to July 31, 2026. The final budget period allocates $142,619.00 for salaries and fringe, $3,500.00 for supplies and current expenses, and $2,500.00 for travel. Invoices must be submitted quarterly according to specified deadlines corresponding to four-month service periods, and each invoice must include contractor information, invoice number, state contract number, service descriptions, and authorized signature. The project's desired outcome is to increase equitable access to inclusive and culturally appropriate healthcare, with the primary performance measure being the number of adults with disabilities who experience fewer unmet preventive health needs, including mental health services. An annual high-level report using a provided template is due by September 30, 2025.
