About this file
This is a Debarment and Suspension Certification document issued by the Cherokee County School District (CCSD) Procurement Services Department in Georgia for an RFP soliciting weight room and training equipment for district high schools. The certification requires all bidders and their subcontractors to confirm that they are not debarred, suspended, ineligible, or excluded from contract awards by the U.S. Department of Agriculture or any other Federal, State, or Local Agency. Bidders must execute this certification under penalty of perjury as a condition of bid consideration, and any bidder unable to provide the certification will be deemed ineligible for contract award. The document specifies that bidders must immediately notify CCSD if they become debarred or placed on the Consolidated List of Debarred, Suspended, and Ineligible Contractors during the bid process or following contract award.
The certification document requires bidders to provide their legal company name, corporate officer signature, and title as part of the submission. All responses will be verified by CCSD prior to award consideration. This requirement aligns with Federal Acquisition Regulation (FAR) 48 C.F.R. Chapter 1 Subpart 9.4 and serves to protect the government's interests by ensuring that only responsible and eligible contractors are considered for the weight room equipment contract, which includes delivery, installation, and assembly services at multiple school locations within the district.
