Q1- Reference: Instructions for the Submission of Proposal Information Q1- Q3 (d.) COMMERCIAL PRICE LIST: Offeror shall submit their current, dated, published commercial price list of all items and maintenance offered in the proposal. This includes price list catalogs or other documentation setting forth the prices charged to the general public. Is this a full commercial price list of offered items? Isn’t this what is provided in the Contract Price Book?
Answer Q1 – Yes. The Commercial List Price should be submitted with proposal package.
Q2 - (e) DISCOUNT & PRICING INFORMATION WORKSHEET Each offeror shall submit a completed Discount & Pricing Information Worksheet (Attachment 3), providing information that supports the discount(s) offered. What kind of information are you looking for here? Is this what should be provided on pages 5-8 of Agreement? This is a new document than what was provided in our past proposals titled the same.
Answer Q2 – Attachment 3 only applies for quoting Government requirements after award and no action is needed as part of this RFP. No, this is not the same information to be provided on pages 5-8. This document was not included under the past solicitation.
Q3 - (o.) PAST PERFORMANCE INFORMATION: If an offeror does not have an existing Radiation Therapy Systems Contract or did not have one in the past 12 months, the offeror must provide a list of their three largest commercial customers, in terms of dollar value, over the past 12 months. Vendors, who currently have an existing Radiation Therapy Systems Contract or had one in the past 12 months, do not have to submit past performance information as the Government will utilize internal past performance information. The Government reserves the right to request additional commercial customer information in the event additional information is needed to evaluate an existing or past program contract holder. Need clarification – if we currently have an existing contract do we have to submit past performance information?
Answer Q3 – Amendment 0001 addressed this question.
Q4 - We looked at the “Instructions for Submission of Proposal Information” section of the solicitation. However, we noticed that a deadline time was not listed with the due date. Is there a time by which the proposal needs to be submitted by (i.e. 8 AM on 6/3 or 5 PM on 6/3)?
Answer Q4 – This was inadvertently omitted. SF1449 section (8) Offer Due Date/ Local Time: 06/03/2019 3:00PM Central Time Q5 – Would you like quotations to be provided with proposal submissions?
Answer Q5 – No. Quotations are not required to be submitted at proposal submission time. It will be requested during the proposal evaluation period by the contract specialist reviewing the offerors proposal.
Q6 - Pgs 28 & 29 – Part Three – Training 3-1 Operator Training: Please clarify last statement of Sect 3-1 Operator Training. Does the VA expect any required off-site training to be covered/paid for by the contractor? Does this include travel expenses of VA employees?
Answer Q6 - Initial on-site Operator Training will be included, all off site training will be tuition only, no travel, lodging or per diem are to be included.
Q7 – Pg. 5 Sec 1 LINEAR ACCELERATOR SYSTEMS. Please clarify both the HIGH & LOW Energy Base system description. Is VA expecting a system that provides Photon energy levels of 6 AND 18 MV or 6 THROUGH 18 V? Additionally, is the MeV description the same as MV? i.e. ‘nominal energies of 4-21 MeV’ Answer Q7 - The base system can be either single nominal energy (MV) photon beam or multiple nominal energy photon beams with several nominal electron energies (MeV). Both types of Systems are acceptable. It can be 6MV or dual energy (6MV and 18 MV).
Q8 - On Attachment 5 CONTRACT PRICE BOOK Should I focus on the ‘Catalog’ tab now and use the ‘Offer-Mod Request’ going forward if we need to make changes? Or Do you want me to duplicate the info on both the ‘Catalog’ and ‘Offer-Mod Request’ tabs? The columns look the same on both tabs.
Answer Q8 – For the proposal submission only the “CATALOG” tab should be populated. The “OFFER-MOD-REQUEST” tab is for after award changes to the contract.
Q9 – Reference Q9 – Q11 Part 5 Information Technology Requirements.
Section 8 Security Controls Compliance Testing. The Contractor Rules of Behavior, Appendix E and Cybersecurity training were not posted.
Answer Q9 – Due to the size of VA Handbook 6506.6, we are unable to attach it as part of the solicitation. Appendix E can be detached and completed from this website: https://www.va.gov/vapubs/viewPublication.asp?Pub_ID=471&FType=2
Q10 - In Section “3. VA INFORMATION CUSTODIAL LANGUAGE,” line item “i” states the following:
0. The contractor/subcontractor's firewall and Web services security controls, if applicable, shall meet or exceed VA's minimum requirements. VA Configuration Guidelines are available upon request. Can you post to fbo.gov?
Answer Q10 – VA configurations will be provided after award.
Q11 - In Section “4. INFORMATION SYSTEM DESIGN AND DEVELOPMENT,” line item “g (2)” states the following: (2) Include this Privacy Act clause, including this subparagraph (3), in all subcontracts awarded under this contract which requires the design, development, or operation of such a SOR.
It appears that subparagraph (3) is missing from the document.
Answer Q11 - Subparagraph (3) is found in h (3) “Systems of Records”.
