Overton Brooks VAMC Infection Prevention & Control Construction Risk Assessment (ICRA)

*A preliminary ICRA is required for contracting purposes only; do not post during construction. The FINAL ICRA must be posted at the construction site.

Project: NFS Refrig, Freezers & Controls 667-18-115 Preliminary Assessment* by: Vickie L. Moore Date: 1/25/19 FINAL ICRA Determined and Processed by: ______________________________Final Date: ________________

STEP ONE: CIRCLE the Type of Construction Project based on scope and expected activity (Type A-D)

TYPE

A

Inspection and Non-Invasive Activities. Includes, but is not limited to:

removal of ceiling tiles for visual inspection only, e.g., limited to 1 tile per 50 square feet painting (but not sanding) wallcovering, electrical trim work, minor plumbing, activities which do not generate dust or require cutting of walls or access to ceilings other than for visual inspection.

TYPE

B

Small scale, short duration activities which create minimal dust Includes, but is not limited to:

installation of telephone and computer cabling access to chase spaces cutting of walls or ceiling where dust migration can be controlled.

TYPE

C

Work generates a moderate/high level of dust or requires demolition/removal of any fixed building components or assemblies Includes, but is not limited to:

sanding of walls for painting or wall covering removal of floorcoverings, ceiling tiles and casework new wall construction minor duct work or electrical work above ceilings major cabling activities any activity which cannot be completed within a single work shift.

TYPE

D

Major demolition and construction projects Includes, but is not limited to:

activities which require consecutive work shifts requires heavy demolition, removal of a complete cabling system, or excavation new construction.

STEP TWO: CIRCLE the affected Patient Risk Groups. If more than one risk group will be affected, select the higher risk group.

Overton Brooks VAMC Infection Control Precautions for Class III Projects

Based on the Infection Control Risk Assessment of this construction/remodel project, the following activities are required.

Project: NFS Refrigerator, Freezer & Control #667-18-115 for BN10 Dry Storage ONLY

During Construction Project: Upon Completion of Project:

C la ss

II

I

1. Complete all critical barriers i.e. sheetrock, plywood, plastic, to seal area from non-work area before construction begins. Construct the BN10 barrier with no entrance to the site.

2. Keep doors to the project area closed.

3. Block off and seal air vents if the HVAC system is removed or isolated.

4. Place dust mat at entrance and exit of work area

5. Remove or Isolate HVAC system in area where work is being done if required to prevent contamination of adjacent areas.

6. Maintain negative air pressure within work site with filtration and pressure monitoring. Utilize HEPA equipped air filtration units if air is recirculated within the building.

7. Construction Waste/Debris

a. Transport in tightly covered container,

b. Containers without tight fitting solid lids must have covering taped in place BEFORE transport outside the project area,

c. Outside of containers must be cleaned when visibly soiled, and

d. When removing construction debris, utilize only the designated construction route.

8. Minimize construction worker traffic within the facility.

9. Provide appropriate secure temperature controlled storage for refrigerated/frozen food and pharmaceutical storage.

10. Provide for safe protected transport of pharmaceuticals and bulk refrigerated and frozen food from storage area to production areas.

1. Wipe work surfaces with cleaner/disinfectant.

2. Contain construction waste before transport in tightly covered containers.

3. Periodically clean outside of construction waste transport container.

4. Do not remove barriers from work area until completed project is inspected by Engineering Service, Infection Prevention and the area is thoroughly cleaned.

5. Remove barrier materials carefully to minimize spreading of dirt and debris associated with construction.

6. Wet mop area with cleaner/disinfectant.

7. Upon completion, restore HVAC system where work was performed.

Maintain control measures until ALL Work is completed. Exceptions must be approved by Infection Prevention on a case by case basis.
