Amendment #3 DATED 9-19-2022

Kellyann Bruno Contracting Officer The purpose of this modification is to answer RFI question for IFB 36C78622B025

1. Monument Markers- Standardly in Section 32, no Section Markers are mentioned in the Specifications but on Drawing LS-101 it shows a list of Section Monuments on the Layout Table. It shows 21 Markers and on Drawing LS-302 and its shows Drawing Details. Are any of the 21 Markers existing and why are they not listed in the Specifications.

Answer: The Section Monuments table on LS-101 indicates new monuments. Section monuments are addressed in NCA’s standard specification 10 14 00.

1. Concrete Precast Post shown on Drawing LS303 are listed on Drawings but no mention in the Specifications Answer: Precast concrete is addressed in 04 72 00. A specific product is specified for the Utility Post.

1. There are no specifications or drawings pertaining to or showing the precast concrete vaults. These are the in-ground concrete vaults that would house the casket or cremains urns at the time of service, which are generally preplaced in mass quantity during construction of the cemetery. There are no specifications or drawings as to the construction and dimensions of casket/direct bury (DB) vaults or in-ground cremains (IGC) vaults, depth/elevations of their placement, depth of fill above the vaults, and the depth and type of material that should be used as a base material to support the vaults? There is no information regarding these items. Are they part of the scope? If so, please provide specifications/drawing/details for these.

Answer: Vaults are not part of the Scope.

1. Construction Fence: Before construction operations begin, the Contractor shall provide a chain link construction fence, seven feet minimum height, around the construction area indicated on the drawings. Please verify the minimum height of the fence. Also, please confirm that all metal fencing components to receive privacy screen are to be black vinyl coated as referenced in note 7 (FOR "CONSTRUCTION FENCE - SCREEN," PROVIDE BLACK CLASS 2B VINYL COATING ALL METAL PARTS; AFFIX PS – PRIVACY SCREEN PER PRODUCT SCHEDULE TO OUTSIDE OF ENCLOSURE USING MFR'S STANDARD FASTENERS AS NECESSARY TO PREVENT).

Answer: Minimum fence height is 6’. The CONSTRUCTION FENCE - SCREEN refers to the PRIVACY FENCE indicated on LS-10x plans & LS-301. It is vinyl coated, not the balance of CONSTRUCTION FENCE.

1. Coordination of Construction with Cemetery Director through the RE/COR: The burial activities at a National Cemetery shall take precedence over construction activities. Cooperation and coordination with the Cemetery Director, through the RE/COR, in arranging construction schedule to cause the least possible interference with Cemetery activities in actual burial areas is required. Construction noise during the interment services shall not disturb the service. Coordination of work may require that equipment that is heard from the service location be shut-off, or moved out of the area prior to the service, as acceptable to the Cemetery Director through the RE/COR. Please provide the average number of daily internments as this could potentially affect construction production.

Answer: Average of 11 internments per day

1. Per the contract documents, ETO 01 completion is no later than December 30,2023. Please provide the anticipated issuance date of NTP in order to factor in the potential for an accelerated construction schedule for completion.

Answer: The Anticipated NTP will be no later 30 calendar date from award

1. Please provide anticipated monthly nonwork days due to adverse weather.

Answer: The NCA does not forecast nonwork days due to adverse weather.

1. Accreditation Requirements: Testing Laboratory retained and paid for by Contractor must be accredited by one or more of the National Voluntary Laboratory Accreditation Program (NVLAP)programs acceptable in the geographic region for the project. Furnish to the Resident Engineer/Contracting Officer’s Representative (RE/COR) a copy of the Certificate of Accreditation and Scope of Accreditation. For testing laboratories that have not yet obtained accreditation by a NVLAP program, submit an acknowledgement letter from one of the laboratory accreditation authorities indicating that the application for accreditation has been received and the accreditation process has started, and submit to the RE/COR for approval, certified statements, signed by an official of the testing laboratory attesting that the proposed laboratory, meets or conforms to the ASTM standards listed below as appropriate to the testing field. Upon contacting several testing firms in the region, this certification is not held. Will any deviation be allowed from the NVLAP certification if the testing lab complies with all other appropriate testing parameters?

Answer: This is in the text of NCA’s standard spec section 01 45 29. Additionally, NVLAP’s website allows searches for accredited programs/laboratories. A quick search found labs in Tampa that may be suitable. www.nist.gov/nvlap)

1. Prior to commencing work, general contractor shall provide proof that an OSHA certified “competent person” (CP) (29 CFR 1926.20(b)(2) will maintain a presence at the work site whenever the general or subcontractors are present. There is no mandatory Competent Person training or official OSHA certification for a “competent person”. An OSHA "competent person" is defined as "one who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has authorization to take prompt corrective measures to eliminate them" [29 CFR 1926.32(f)]. Please confirm intent.

ANSWER: OSHA 30 is for a superintendent that manages personnel and OSHA 10 is for the worker on site. OSHA 10 (doesn't mean that they couldn't also have OSHA 30), but Superintendent/supervisors must have OSHA 30. work shall be performed in a professional workmanship like manner, adhere to NCA standards, be done in strict accordance with the requirements set forth herein, manufacturer’s requirements, installation guidelines; and be in accordance with applicable Federal, State and Local codes and regulations; inclusive of applicable OSHA Construction Safety Guidelines.

ANSWER: As-built plans will be provided to the awardee.

1. Per the Geotechnical report, groundwater was encountered during borehole advancement at all boring locations. The static depth to groundwater at the site is approximately 3 to 5 feet bls. However, it should be anticipated that the groundwater level will fluctuate due to seasonal climactic variations, surface water runoff patterns, construction operations, and other interrelated factors. Please confirm no other hydrologic data is available from months outside of those in which the geotechnical report was conducted.

ANSWER: The geotech reports commissioned for this project are included in their entirety as appendices in the Project Manual.)

1. In the concrete sidewalk area surrounding the columbaria, is this to be typical grey sidewalk or match the exposed aggregate finish around the current columbaria?

ANSWER: No exposed aggregate sidewalk finishes are in Scope.

1. What material is the proposed 10” Reclaim line extension? DIP or PVC?

ANSWER: Provide DIPS PVC per 32 84 00.

1. Sheet CE-201 shows sediment logs being used for sediment control. Is silt fence an acceptable alternative?

ANSWER: Silt fence conforming to the following is acceptable.

1. LI-001 refers to bid alternate #3. The specification on page 01 00 02-2 B.3 only refers up to bid option 2. Please confirm there should only be two alternates.

ANSWER: Refer to Specifications for Bid Options. There are no “bid alternates.”

1. Note #2 on sheet A-101 mentions exposed aggregate concrete. What are the limits of this concrete?

ANSWER: Disregard. No exposed aggregate sidewalk finishes are in Scope.

1. Section 31 20 00 references KYTC standard specs which I believe are Kentucky details. It also lists CL or ML as suitable material which I do not believe is correct. Should this be FDOT standards and per the geotech for suitable material?

ANSWER: Should be FDOT standards and refer to Geotechnical Report.

1. Plan road details show 08” limerock base for roadways. Geotechnical reports suggest 08’ crushed concrete is acceptable. Please confirm that crushed concrete is an acceptable alternative.

ANSWER: Acceptable to Geotechnical Report

1. There appears to be a clay layer present in the pond to the south that is being expanded. The geotechnical report, addendum #2 states we are able to mix this material with other material for reuse. Addendum #1 states that we should not excavate below the clay layer. 1) Are we able to excavate to the proposed elevation of 3? 2) If we are unable to successfully mix the clay material to make it suitable, how will we be paid for haul off and replacement if necessary?

ANSWER: Adhere to the requirements of the geotech reports/addenda. The intent of pond excavation is to meet on-site fill requirements and preclude the need to import fill. The pond may be expanded as necessary to meet that need, as approved by the COR.

ANSWER: If the appropriate clay material is not available on site, it would likely need to be imported, however there should be no haul-off required in any case. Rather excess material can be stockpiled or spread uniformly at a location acceptable to the Cemetery and as app’d by the COR.

1. In the event of excess fill, are we to stockpile onsite for future use?

ANSWER: See response to question 21, part 2.

1. Please confirm the Early Turnover (ETO) date- Spec 0100 02 1.6.G states 12/30/2023.

ANSWER: Confirmed

1. Can we be provided with a schedule? Anticipated start/completion date?

ANSWER: The period of performance is provided in the IFB.

1. Can we be provided with CADD Files? (Existing and Proposed data lines) ANSWER: CADD files will be provided to the winning bidder.

1. Is there any special earthwork requirement for the following sections?

25. Inground Cremains Interment 4’x4’. (Sections 20-28)

25. Traditional Casketed Direct Bury Interment 6’x10’. (Section 30)

25. Can details/ envelope information be provided for the sections above?

ANSWER: No special compaction requirements

1. Can we be provided with the laydown area sectional detail?

ANSWER: No special sectional requirements. Provide temporary improvements as necessary to allow safe and efficient operations. Restore to pre-construction conditions at close of contract.
