About this file

This document is an Addendum (No. 01) dated July 30, 2025, for the Create Cardiovascular Center project at the Orlando VA Healthcare System located at 13800 Veterans Way, Orlando, FL 32827. The addendum revises the project phasing, reducing the original three phases to two phases: the previous Phases 1 & 2 have been combined into Phase 1, while the previous Phase 3 has been renamed as Phase 2. Key changes include constructing a 2-hour fire-rated temporary egress corridor with ceiling, emergency/egress lighting, and sprinkler system, which must be completed prior to starting construction of the new building expansion shell area adjacent to the stair exit door.

The addendum also addresses specific technical requirements for the project, including maintaining access to an existing toilet room during Phase 1 with temporary construction partitions, and handling distributed antenna system (DAS) and wireless access point (WAP) devices. For DAS devices, contractors must move them above the ceiling, protect them during construction, and reinstall them at the same location in the new ceiling. WAP devices are to be turned over to the VA Contracting Officer's Representative (COR), with the contractor responsible for installing new cabling and the VA to reinstall the devices at new locations indicated in the contract documents. The document includes revised phasing plans and narratives across multiple drawing sheets to reflect these changes.

Addendum No. 01

Date: July 30, 2025

Project: 675-103

Create Cardiovascular Center at Lake Nona

Orlando VA Healthcare System

13800 Veterans Way

Orlando, FL 32827

To: David Mark, VA CO

William Klotzbach, VA CS

Kevin Glover, VA COR

This Addendum supplements and amends the original Drawings and Specifications for the above referenced project. This Addendum shall be forwarded to all Prime Bidders and all appropriate building code authorities. This Addendum shall be taken into account in the preparation of bids and shall become a part of the Contract Documents.

Receipt of this Addendum must be acknowledged on the Bid Form.

This addendum consists of the following items:

1. Project Phasing has been revised to reduce the project into two phases. Previous Phases

1 & 2 have been combined into Phase 1 while previous Phase 3 has been renamed as

Phase 2.

- Revised Phase 1 requires a 2-hr fire rated temporary egress corridor to be constructed with ceiling, emergency / egress lighting and sprinkler to be installed. The temporary corridor must be completed prior to the start of construction of the new building expansion shell area adjacent to the Stair exit door. The temporary corridor may be removed upon the reopening and safe exit discharge from the exterior Stair door.

- Project Phasing Plans and narrative have been revised to reflect this change per VA direction.

2. An existing Toilet room to be removed shall remain in Phase 1 and temporary construction partitions shall be installed to maintain access for patient use. See keynote

1.13 on sheet GC102.

3. Existing distributed antenna system (DAS) and wireless access point (WAP) devices were added to clarify scope for these devices to the contractor.

- DAS devices are to be moved above ceiling and protected during construction; re-installed at same location in new ceiling.

- WAP devices are to be turned over the VA COR. Contractor to install new cabling;

VA to re-install WAP devices at new locations indicated on contract documents.
