About this file

This is a pricing schedule template for an RFQ issued by the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 15 for patient labels at the Shawnee (Leavenworth) CMOP facility. The solicitation number is 36C77026Q0051, with a posting date of February 9, 2026, and original closing date of February 17, 2026 at 3:00 PM Central Time. The RFQ has been extended until January 30th for Line Item 3 only. Vendors must submit a completed SF1449 solicitation cover page, the quote spreadsheet, OEM authorized dealer or distributor documentation, and a Buy American Act certificate to Kelley.Cunningham@va.gov.

The single line item (0003) requires 900 cases of Sprint Label DT400600-3-12TC at the Shawnee (Leavenworth) facility. Vendors must provide the earliest delivery timeframe and ensure compliance with gray market prevention requirements by submitting OEM authorization letters or equivalent documentation from authorized manufacturers or wholesalers. The procurement explicitly prohibits remanufactured or gray market items, and vendors must be OEM authorized dealers or distributors to participate. Any delivery of gray market items constitutes a breach of contract, and the VA reserves the right to terminate the contract or require replacement at no cost to the government.

Gray Market Prevention: Deviation to policy allows non-manufacturing vendors selling supplies to satisfy the Gray Market Prevention language by providing letters of authorization from a manufacturer's authorized wholesaler/reseller via an authorization letter directly from the manufacturer.

(a). Gray market items are Original Equipment Manufacturer's (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM supplies, equipment, and/or services contracts for maintenance of equipment (i.e. replacement parts) for VA Medical Centers. No remanufactured or gray market items will be accepted.

(b). Vendor shall be an OEM authorized dealer or authorized distributor for the proposed supplies, equipment, and/or services contracts for maintenance of equipment (i.e. replacement parts). Verified by an authorization letter or other documentation from the OEM, such that the OEM's warranty and services are provided and maintained by the OEM. All software licensing, warranty and services associated with the supplies, equipment, and/or services contract for maintenance of equipment shall be in accordance with the OEM terms and conditions.

(c). The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right to enforce any of its contractual remedies. this includes termination of the contract or, solely at the VA's election, allowing the vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.
