About this file

This document is a Pricing Schedule (Quote Spreadsheet) for a VA Medical Center contract opportunity soliciting patient labels for the Shawnee (Leavenworth) CMOP (Consolidated Mail Outpatient Pharmacy). The solicitation (RFQ 36C77026Q0051) requires vendors to provide four specific label types with varying quantities: a 2 x 5.5 patient-centric label (1,800 units), a long signature 2.5" x 5.5" label (12 units), a 4 x 6 sprint label (600 units), and a direct thermal packing label (300 units).

The solicitation is issued by the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 15, with a closing date of January 30, 2026, at 3:00 PM Central Time. Vendors must submit quotes via email to Kelley.Cunningham@va.gov and include specific documentation such as the SF1449 solicitation cover page, completed quote spreadsheet, OEM authorized dealer documentation, and a Buy American Act certificate. The solicitation emphasizes strict requirements against gray market items, mandating that vendors provide new OEM supplies with proper manufacturer authorization and maintaining full warranty and service conditions.

Gray Market Prevention: Deviation to policy allows non-manufacturing vendors selling supplies to satisfy the Gray Market Prevention language by providing letters of authorization from a manufacturer's authorized wholesaler/reseller via an authorization letter directly from the manufacturer.

(a). Gray market items are Original Equipment Manufacturer's (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM supplies, equipment, and/or services contracts for maintenance of equipment (i.e. replacement parts) for VA Medical Centers. No remanufactured or gray market items will be accepted.

(b). Vendor shall be an OEM authorized dealer or authorized distributor for the proposed supplies, equipment, and/or services contracts for maintenance of equipment (i.e. replacement parts). Verified by an authorization letter or other documentation from the OEM, such that the OEM's warranty and services are provided and maintained by the OEM. All software licensing, warranty and services associated with the supplies, equipment, and/or services contract for maintenance of equipment shall be in accordance with the OEM terms and conditions.

(c). The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right to enforce any of its contractual remedies. this includes termination of the contract or, solely at the VA's election, allowing the vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.
