About this file

This file is an Amendment/Modification (Standard Form 30) and Questions and Answers document for a federal Request for Quotation (RFQ) for ice machine preventive maintenance services at Department of Veterans Affairs facilities within Veterans Integrated Service Network 22 in Arizona.

The solicitation, numbered 36C26227Q0030, requires preventive maintenance services for 62 Follett ice machines located across multiple VA facilities, including the Main Campus, Southwest Clinic, Show Low Community-Based Outpatient Clinic (CBOC), and Globe VA Clinic. The requirement is for annual preventive maintenance services only; repairs, emergency maintenance, and replacement parts are excluded and will be handled by the VA HVAC Shop. Offers are due November 10, 2026, at 4:00 PM Mountain Standard Time. The contract period of performance runs from January 1, 2027, through December 31, 2027. The Service Contract Labor Standards statute applies, with work at the Show Low CBOC subject to Navajo County, Arizona Wage Determination 2015-5477 Revision 32. No bid bond or offer security is required. Attendance at the October 27, 2026 site visit is optional. Offerors may include qualifications and past performance information of proposed subcontractors, which should be clearly identified and distinguished from the prime contractor's experience. Payment for preventive maintenance services will be contingent upon Government review and acceptance of satisfactory performance, with invoices submitted following completion of applicable services.

Answers to Questions: Ice Machine Solicitation RFQ 36C26227Q0030_1

1. Which offer due date governs: November 9, 2026 (Section E addendum) or November 10, 2026 (SF 1449)? Offers are due November 10, 2026, at 4:00 PM MST

2. Are breakdown repairs between preventive maintenance visits in scope? There is no repair CLIN in the price schedule. If repairs are in scope, how are they ordered and paid? No. This requirement is for preventive maintenance services only. Repairs identified during preventive maintenance services shall be reported to the Government. Emergency repairs, corrective maintenance, replacement parts, and breakdown repairs are not included under this requirement and will be handled by the VA HVAC Shop.

3. Please provide the full equipment inventory with model, serial number, and age for all 61 machines, including the Show Low unit address. Please also confirm the Main Campus rating (CLIN 0001 states 550 lb/day; the SOW states 500 lb/day) and the Southwest Clinic rating (listed as 6 lbs/day). The equipment inventory is included in the Statement of Work and identifies all covered Follett ice machines by location. The requirement includes 62 machines, including the Globe location under CLIN 0011. No additional inventory information will be provided. Any necessary clarification regarding equipment production ratings will be addressed by amendment, if applicable.

4. What does the CLIN 0011 unit price (10 EA, additional machines) represent: annual preventive maintenance per added machine, or per visit? How will additions be ordered? Pursuant to Amendment 0002, CLIN 0011 has been revised to include the Globe VA Clinic location and its one (1) Follett ice machine. The Globe VA Clinic is located at 5860 S. Hospital Dr., Suite 111, Globe, AZ 85501. Offerors shall price CLIN 0011 in accordance with the requirements identified in Amendment 0002 and the applicable preventive maintenance requirements set forth in the Statement of Work.

5. Please confirm the billing trigger. SOW 18 refers to invoicing after a patient is discharged and payment for storage containers, while B.1 states invoicing upon completion of satisfactory work. Is billing per visit, per quarter, or per site? The references in SOW Section 18 regarding patient discharge and storage containers are not applicable to this requirement. Invoices may be submitted following completion of the applicable preventive maintenance services. However, payment is contingent upon Government review and acceptance of satisfactory performance.

6. Please attach wage determination 2015-5469 Rev 32 (referenced by link only) and confirm whether the Service Contract Act applies. The Service Contract Labor Standards statute applies to this requirement. Work performed at the Show Low CBOC is subject to the applicable Service Contract Labor Standards wage determination for Navajo County, Arizona, Wage Determination 2015-5477 Revision 32. The applicable wage determination may be found here: https://sam.gov/wage-determination/2015-5477/32.

7. Is attendance at the October 27, 2026, 10:00 AM Arizona-time site visit mandatory for quotation responsiveness? If so, may a properly authorized representative of a named first-tier subcontractor attend on behalf of the SDVOSB prime? No. Attendance at the site visit was not mandatory and is not a requirement for quotation responsiveness. Offerors may elect to attend at their discretion.

8. The RFQ requests relevant technician qualifications, experience and past performance. Can a proposed qualified, named first-tier SDVOSB subcontractor's technical staff, licenses, certifications and prior contracts be included and considered as that firm's experience, separately identified from Bronzeway's own history? Or must the prime itself meet a specified minimum years-of-experience threshold? Offerors may submit the qualifications, experience, certifications, licenses, and past performance information of proposed subcontractors as part of their quotation. Any subcontractor information should be clearly identified and distinguished from the prime contractor's experience. There is no specified minimum years-of-experience requirement for the prime contractor identified in the solicitation.

9. Is there any bid bond, bid guarantee, or other offer security required with the RFQ quotation? No. A bid bond, bid guarantee, or other form of offer security is not required under this RFQ.
