About this file
This document is an Inquiry Responses file for solicitation 36C26225Q0847, addressing two key questions about backflow device certification. The responses clarify that the backflow prevention devices to be serviced are a mixture of fire suppression, domestic water, and industrial water systems. Regarding documentation, the City of Long Beach Utilities Department will provide pre-filled inspection sheets for two specific backflow devices located at the NE corner of E. 7th Street and Bellflower, and at building 125 pumphouse at Gate 3 on Bellflower. For the remaining devices, the city will provide a sample form that the contractor must complete, typed for legibility, and submit with either a wet or digital signature to both the VA and the City of Long Beach.

36C26225Q0847 INQUIRY RESPONSES:
Q1: Are there fire protection system backflow preventers involved, or are they all domestic backflow preventers to be inspected, or a mix?
- The backflow prevention devices to be serviced are a mixture of fire suppression, domestic water, and industrial water.
Q2: Will the VA or City of Long Beach (Water Department or Health Department) be providing pre-filled or standardized forms for the backflow testing submissions, or is the contractor expected to identify each device, manually complete the documentation, and submit it independently?
- The City of Long Beach Utilities Department provides a pre-filled form or backflow testing at the two main water connections (NE corner of E. 7th Street and Bellflower, and the building 125 pumphouse at Gate 3 on Bellflower), in the past the contractor obtained this form from the City, completed the form, and submitted copies to both the VA and the City of Long Beach.
- The City of Long Beach will provide prefilled inspection sheets for the two (2) backflow devices previously mentioned. For the rest of the devices the city provides a sample form to be filled out by the contractor (typed for legibility) with a wet or digital signature.
