About this file
This document is a Quality Assurance Surveillance Plan (QASP) that establishes performance standards and monitoring requirements for support services under a Performance Work Statement (PWS).
The QASP identifies three critical performance objectives with zero-tolerance thresholds. The first objective addresses inspection service frequency and intervals, requiring 100% compliance with certification testing intervals as specified in PWS Section 2 and VA Directive or regulatory requirements, with 100% inspection methodology to verify achievement. The second objective concerns reports, mandating that all reports be accurate, complete, and submitted on time with zero discrepancies allowed, subject to 100% inspection verification. The third objective requires use of qualified personnel, stipulating that 100% of personnel must meet CETA certification requirements as outlined in the PWS with no deviations permitted, also subject to 100% inspection. The plan explicitly states that failure to meet these standards will result in termination for default. Each performance objective includes defined standards, maximum allowable degrees of deviation, and corresponding surveillance methods, with all three requirements allowing no deviations from established thresholds.

ATTACHEMENT 1
Quality Assurance Surveillance Plan (QASP)
The following Performance Requirement Services Summary identifies the performance objective and performance thresholds for critical tasks associated with providing support services for this requirement. The performance threshold describes the minimum acceptable levels of service required for each requirement. The Performance
Requirements Summary shall not take the place of the PWS. These thresholds are critical to mission success.
Failure to meet the standard will result in termination for default.
