About this file

This is a Quality Assurance Surveillance Plan (QASP) for home oxygen and ventilator services at the VA Pacific Islands Health Care System under Solicitation 36C26126Q0452. The QASP establishes performance monitoring standards and metrics that contractors must meet throughout the contract period, which spans five one-year ordering periods from October 1, 2026, through September 30, 2031. The plan defines specific performance indicators across nine key categories: Order Processing & Timeliness, Home Visits & Assessments, Patient Safety, Equipment & Maintenance, Staff Qualifications & Competency, Reporting, After-Hours & Emergency Coverage, and Discontinuation & Transition.

The QASP establishes rigorous service requirements including order confirmation within one working hour, same-day or next-day initial setup depending on prescription receipt timing, 90-day home visits for oxygen patients and 30-day visits for ventilator patients, and 24/7/365 emergency response capability with 30-minute telephone response and two-hour on-site service. Contractors must maintain preventive maintenance schedules, provide comprehensive patient education and safety assessments, ensure all clinical staff hold relevant licenses with minimum two years of home care experience, and submit monthly contract usage reports and quarterly reports on customer satisfaction, infection control, performance improvement, and fall prevention. The Contracting Officer's Representative (COR) monitors compliance through electronic logs, home visit reviews, staff personnel files, and periodic after-hours service testing, with most performance metrics requiring 90-95% compliance levels.

QUALITY ASSURANCE SURVEILLANCE PLAN

FOR

HOME OXYGEN AND VENTILATOR SERVICES

VA Pacific Islands Health Care System (VAPIHCS) Solicitation 36C26126Q0452 This Quality Assurance Surveillance Plan (QASP) has been developed to evaluate contractor performance while implementing the Performance Work Statement (PWS) for Home Oxygen and Ventilator Services. It is designed to provide an effective surveillance method for monitoring contractor performance against the objectives and timelines established in the PWS.

The Contractor, and not the Government, is responsible for management and quality control actions to meet the terms of the contract. The role of the Government is to monitor quality assurance to ensure contract standards are achieved. The Contractor is required to develop a comprehensive program of inspections and monitoring actions.

This QASP aligns directly with the PWS for Solicitation 36C26126Q0452 and includes ventilator-specific surveillance metrics, order processing timelines, home visit cadence requirements, and quarterly reporting requirements as specified in the PWS.
