AMENDMENT NO. 0012
Solicitation 36C26118R0541 Project 654-18-831 IPP USP 797/800 Compliance (Pharmacy)
General
The following changes, additions or deletions shall be made to the following Contract Documents.
All other conditions shall remain the same.
1. SPECIFICATIONS
a. N/A
2. DRAWINGS
a. N/A
3. OFFEROR QUESTION AND CLARIFICATION
a. Question: We would like to request an additional site visit for our engineers to walk the existing space and see the temporary location for the temporary trailer and where it would connect to existing water and power. We would like to request a specific date for our out of state engineering firm that will be in town for another project of yours. We would like to request November 29th 2018 after 10:00 am if at all possible so we don’t cause them conflict with their other meeting on site?
b. Response: As per Amendment 0006, clause 52.236-27 (Construction) (FEB 1995), site visits may be arranged during normal duty hours by contacting Dan Davis (COR for this project). A Site Visit is scheduled with Dan Davis for 29 Nov 2018 after 10:00am PST.
c. Question: Addendum #9 references a "swing space" but the solicitation's verbiage mostly references "install temporary leased USP 797/800 compliant pharmacy compounding trailer or equal temporary removable facility." Does the "Swing Space" imply there is an existing space within the hospital that is currently 797/800 complaint we could use temporally to relocate the people in the existing space to while construction is in process? or would the "swing space need to be brought up to compliance including equipment needed to be use?
d. Response: There is no 797/800 compliant swing space. The building space available will need to be converted to a compliant pharmacy work space, complete with all utilities and hoods. That will also include certification by a third party.
e. Question: Addendum #9, page 3 item #1.3 "Estimated Construction Contract price (design -to limitation): $350,000" Does this amount include the amount of the
"Temporary swing space pharmacy trailer " that is to be "turn key installation" cost?
f. Response: Yes
g. Question: 1.5. The DB shall provide one proposal in three sections: a design proposal and a construction proposal for the remodeled space. A proposal for the temporary trailer (temporary swing space) and the site preparations to install the trailer (temporary swing space).
1.5.1. Design proposal (AE) shall be a complete and detailed fee proposal for completing all field investigation, design analysis, construction drawings, specifications, construction period services, inspections, testing, and other services required under this Statement of Work. Use the form provided by the VA.
1.5.2. Construction proposal shall be presented in a schedule of values format to be utilized during the construction portion.
Please provide form reference for bidding as we have not found on the FBO website per the above highlights?
In section 1.5.2 construction proposal in a schedule of values format to be turned in at time of bid? without the full design plans we feel this would be difficult to provide at this time.
h. Response: Turned in prior to start of construction.
i. Question: Under “Scope” on page 3 the “specific design and construction elements include” does not mention to what extent the architectural items need to be redone or modified. To what extent does the VA anticipate the walls, ceiling, flooring, doors, structural items, etc. will be affected?
j. Response: Those areas that will need to be redone or modified as identified during the design period.
k. Question: “2.2.3. Development of a complete stamped and signed design package including all appropriate trades which, at a minimum, should include: architect, civil, structural, mechanical, electrical and plumbing for temporary pharmacy trailer (temporary swing space) site preparation and installation of trailer (temporary swing space).”
After speaking to many “turn Key trailer” providers and our engineering firm it has been brought to our attention that this statement is not how it would be provided. Our engineers would not be stamping these drawings that are provided directly from the trailer providers. The trailer providers would be the only ones stamping their own drawings, certifying their trailer, setting up and cleaning and tearing down, as well as providing training to personnel that would be working in the unit. This is very similar to how a metal building supplier would provide drawings stamped by them. We request this statement be modified to reflect this.
l. Response: Ok, if the trailer option is used.
m. Question: If a modular trailer is used for the temporary facility, Can the Temporary water and electrical hook ups be above ground as much as possible covered by raised bumper cover system and the waterline insulated as needed? Or would this service have to be put underground?
n. Response: Above ground is acceptable as long as it doesn’t interfere with patient care or foot traffic.
o. Question: For the discharge of water used in the temporary modular trailer would this need to be connected to the hospital drains in back of the emergency room ambulance entrance’s sanitary sewer system underground? Or would a holding tank with a pump service be used?
p. Response: Must be connected to drains or contractor provided tank with service.
