1. SF1442 block 8 states, “ADDRESS OFFER TO:

00100 INSTRUCTIONS, CONDITIONS AND OTHER STATEMENTS TO OFFERORS C. 1. States to address to:
Department of Veterans Affairs VA Sierra Pacific Network (VISN 21) ATTN: Timothy Bertucco 201 Walnut Avenue Mare Island, CA 95492
Please verify address location to submit proposals.
Answer: The correct address is the location listed in 00100 Instructions, Conditions and Other Statements to Offerors. Please do NOT mail the proposals to the location in Block 8 of the SF 1442. Please see revised Block 8 of the SF 1442 attached.
2. Attachment C included in the solicitation package “SUBCONTRACTOR/TEAMING
PARTNER CONSENT FORM FOR THE RELEASE OF PAST AND PRESENT PERFORMANCE
INFORMATION TO THE PRIME CONTRACTOR,” pertaining to “00110 EVALUATION OF OFFERORS PART II. 2. EVALUATION FACTORS Factor 2 – Recent, Relevant Experience of the Firm, Factor 3 – Past Performance on Recent, Relevant Projects” is the VA allowing major subcontractor/teaming partners to have past flow performance for experience and past performance on this solicitation?
Answer: Please refer to Factor 2 – Recent, Relevant Experience of the Firm where you may submit experience of the prime contractor. Attachment C will apply only to JVs.
3. Solicitation Page 10 of 137 | Hard Copy Submission Mailing Address | “An original and five (5) copies of the technical proposal for Factors 1, 2, and 3 and an original of the Administrative volume shall be submitted to: Department of Veterans Affairs, VA Sierra Pacific Network (VISN 21), ATTN: Timothy Bertucco, 201 Walnut Avenue, Mare Island, CA 94592.”
0. It is mentioned in the solicitation, and was also mentioned during the pre-proposal conference, that we are to submit hard copies to the address featured on page 10 of the solicitation. However, this address conflicts with the address featured on the front of the SF 1442 in block 8. Please confirm if hard copies are to be submitted to the Mare Island, CA address or the Honolulu, HI address.
Answer: The correct address is the location listed in 00100 Instructions, Conditions and Other Statements to Offerors. Please do NOT mail the proposals to the location in Block 8 of the SF 1442. Please see revised Block 8 of the SF1442 attached.
4. Is a teaming agreement or a key subcontractor agreement allowed for this RFP? If so, is their past performance acceptable to meet the requirements of this solicitation?
Answer: Please refer to the direction stated in Factor 2, Recent, Relevant Experience of the Firm. The offeror may submit a maximum of five (5) construction projects in which the Offeror (or JV Partner) was the Prime Contractor.
5. Solicitation Page 36 of 137 | Factor 3, Past Performance | “If a completed Contractor Performance Assessment Report (CPAR) evaluation is available, it shall be submitted with the proposal for each project included in Factor 2 for construction experience.”
a. If a CPARS evaluation exists for a particular project submitted under “Factor 2, Recent, Relevant Experience of the Firm”, please confirm if that CPARS evaluation can be submitted in lieu of a PPQ, or if it is to be submitted in addition to the PPQ.
Answer: PPQs are not mandatory. However, in many instances CPARs contains ratings without narratives or added detail. The PPQs, if narratives are provided, could help to supplement the CPARs ratings.
6. You indicated in your pre-proposal teleconference that we are to include in Factor 3 – past performance Attachment B along with including any CPAR evaluation in our proposal. You indicated that it would be a positive if both were received by the government. It has been our experience that our contracting officer’s will not complete a past performance questionnaire if an official CPARS evaluation exists. Would the government please consider allowing offerors’ to submit CPAR evaluations in lieu of Attachment B?
Answer: You will not receive an adverse rating for failing to have PPQs. PPQs are not mandatory. However, in many instances CPARs contains ratings without narratives or added detail. The PPQs, if narratives are provided, could help to supplement the CPARs ratings.
7. On page 36 of 137 of the RFP under Factor 3, Past Performance (2) Adverse Information the RFP states “Submittal of quality performance indicators or other management indicators that clearly support than an Offeror has overcome past problems is required.” It is unclear to us that if an offeror has not received any letters of concern, cure and or show cause notices or Terminations for cause/default within the last three years if we are required to provide information under this section. Can the government please confirm that if the offeror has received no letters of concern, cure and or show cause notices and/or termination for cause /default within the last three (3) years that no information is required under this section of our proposal?
Answer: If you have no adverse information, please explicitly state under Factor 3 section 2 that you have no adverse information. JVs with less than 3 years of experience from the date of the proposal submission should submit adverse information from each company that is a member of the JV. Please see revised Factor 3 of the solicitation attached.
8. On page 37 of 137 of the RFP under Factor 3, Past Performance (3) Organizational Structure Change History the RFP states “To facilitate this relevancy determination, Offeror shall include a “roadmap” describing any/all such changes in their organization.” It is unclear to us that if an offeror has not been acquired, acquired another company, merged with another company and/or reorganized their division, business groups, subsidiaries, etc. if we are required to provide information under this section. Can the government please confirm that if none of these organizational structure changes have occurred to an offeror that no information is required under this section of our proposal?
Answer: If the section is not applicable, please explicitly state under Factor 3 section 3 that the section does not apply. Please see revised Factor 3 of the solicitation attached.
9. On page 33 of 137 of the RFP under Factor 1, Safety (2) certification it states “provide a certification that you have no more than three (3) serious, or one (1) repeat or one (1) willful OSHA or any EPA violation(s) in the past three years. It is unclear what “certification” the government will require for this section. Will a statement from the offeror “certifying” that it has no more than three (3) serious, or one (1) repeat or one (1) willful OSHA or any EPA violation(s) in the past three years suffice?
Answer: Please submit your written certification on company letterhead and signed by a principal of the company. If submitting as a JV, please insure the certification is signed by a principal of each company that is a member of the JV. JVs with less than 3 years of experience from the date of the proposal submission should submit certifications from each company that is a member of the JV and signed by a principal of each company.
10. Safety narrative to be limited to two (2) pages. Please verify double spaced two (2) pages.
Answer: The safety narrative page limitation is revised to three (3) pages. Please see the Solicitation’s Instructions to Offerors Section D number 5 for spacing and font size. Please see revised Factor 1 Safety section (i)(3) attached.
11. Is there the last day for submission of RFI’s?
Answer: Please refer to Block 10 on the SF 1442.
