Key personnel requirements include a housekeeping supervisor with a minimum of five years' experience (at least three in general housekeeping and two in hospital housekeeping) available 24/7, and shift leaders with at least one year of recent hospital housekeeping experience meeting industry certifications such as IEHA. All employees must pass security clearances, background checks, immunizations (TB, Hepatitis B, influenza, MMR, Varicella, and Tdap), and pre-employment physical examinations at the contractor's expense. HOUSEKEEPING PERFORMANCE WORK STATEMENT

1.1. CONTRACT TITLE: Housekeeping Services for Veteran Affairs George E. Wahlen Medical Center Campus (VASLC), which includes main hospital Building 1 and 14, Buildings (2,4,5,7,8,16, 18, 42, 47, and 49).

1.2. GENERAL: This is a non-personnel service contract to provide housekeeping services. The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn, is responsible to the Government.

1.3. DESCRIPTION OF SERVICES/INTRODUCTION: The Contractor shall provide all labor, supervision, management support, transportation, equipment, materials, and non- personal services necessary to provide complete hospital housekeeping, general housekeeping, and Regulated Medical Waste (RMW) collection as defined in this Performance Work Statement (PWS). The Contractor shall perform to the standards in this contract.

Technical Exhibits specify housekeeping services required to curtail the spread of healthcare acquired infections and to maintain sanitary standards established for the listed facilities belonging to the VASLC. The Contractor shall perform all required services in a sanitary, safe, and aesthetically pleasing condition.

All references to locations and functions of rooms and areas are as they will exist at contract commencement date. Such references do not necessarily indicate that these locations or functional designations will remain stable and may be changed by the Contracting Officer (CO) when necessary to adjust for mission requirements. Where specified, performance measurements and standards may be used to achieve a variety of goals, to include but not limited to, the collection of data to test the practicability of a measure or standard, to identify less than required compliance, and to collect data in support of "quality assurance."

The Contractor shall also be prepared to present a briefing regarding the way he/she intends to accomplish contractual obligations. Technical statements are required with a technical review as part of the evaluation process. Contractor shall address the technical evaluation factors in the presentation prior to award.

1.4.1 Hours of Operation-

The Contractor shall perform services required by this Performance Work Statement (PWS) beginning Sunday, Monday, Tuesday, Wednesday, Thursday, Friday, and Saturday.

The contractor shall perform weekend services on Sundays and Saturdays required by this PWS:

The Contracting Officer Representative (COR) must approve any changes to the Contractor’s work schedule in advance.

1.5.STAFF AND WORK SCHEDULES:
1.5.1.Key Personnel: Includes Supervisor and Shift Leaders. Key personnel should be available, via telephone or pager, 24 hours a day, 7 days a week.
1.5.2.Standard: Housekeeping Supervisor Absence from Duty: If the Supervisor is absent from a normal 8-hour shift, the Contracting Officer or his/her designated representative shall be notified orally by the Contractor no later than 2 hours after commencement of the designated 8-hour shift. For scheduled absences of a longer duration, the Contractor shall notify the Contracting Officer or his/her designated representative in writing at least 4 calendar days before any scheduled absence. Extended absences or the ongoing use of temporary personnel resulting in inadequate managerial oversight throughout VASLC may constitute nonperformance.
1.5.3Standard — Experience Requirements: The supervisor shall possess a minimum of 5 years’ experience, which includes at least 3 years general housekeeping experience and 2 years of specific hospital housekeeping experience.
1.5.6.Task —Shift Leader Staffing: The Contractor shall ensure a shift leader is present for each scheduled shift of employees to execute work requirements. The shift leader(s) shall be fully capable of directing housekeeping staff to perform emergency cleaning when required. Emergencies consist of but are not limited to accidental discharge of the sprinkler system, leaking roofs, and plugged toilets.
1.5.7.Standard- Certification and Experience Requirements: Shift Leaders shall possess at least 1 year of experience within the last 3 years in hospital housekeeping services and meet or exceed comparable industry certifications (IEHA, etc.), training, and experience requirements to provide required service in a full-service outpatient clinic. Shift Leaders shall comply with industry, State, or Federal continuing education requirements.
1.5.8.Standard: All proposed substitutions of Shift Leaders shall be submitted in writing to the Contracting Officer's Representative (COR) at least 15 calendar days prior to the proposed substitution. All proposed substitutes shall possess at least qualifications equal to the qualifications of the person being replaced.
1.5.9.Standard: The Contractor shall provide designated key personnel who shall be immediately available for the Government to consult with 24 hours per day, seven days per week. The unavailability of key personnel to resolve contractual issues constitutes non-performance under the contract.
1.5.10.Standard: Certifications technical capability shall be submitted at time of proposal for Housekeeper and Shift Supervisor/Leaders.
1.5.11.Standard: Contractor shall provide staff consistent with all applicable housekeeping requirements in accordance with identified herein.
1.5.12.Standard: The Government reserves the right to verify and confirm all certifications and technical capability provided, including: (a) name and address of the health care facility; (b) certifications or memberships and/or completed courses cited above; and (c) copies of the formal training programs considered comparable to the IEHA curriculum (if not previously furnished).
1.5.13.Standard: The Contractor agrees to assign to the contract tasks those persons whose resumes were submitted with its proposal and who are necessary to fulfill the requirements of the contract as "key personnel". No substitutions may be made except in accordance with this clause.
1.5.14.Standard - Work Scheduling: The Contractor shall provide to the COR monthly all work schedules at least 30 calendar days prior to month of accomplishment. The Contractor shall appropriately schedule work/phasing to minimize impact on routine Government operations (all areas) and patient traffic flow. Schedules shall include the date, show the phasing of work by room/area/building/wing throughout the work period, and show the schedule of completion of each task requirement.

1.5.15 Standard - Schedule Changes: The Contractor shall submit written schedule changes to the COR at least 24 hours in advance of beginning the output.

1.5.16. Standard — Personnel Roster: The Contractor shall submit a copy of its personnel roster to the COR for all personnel working in the facility no later than 30 calendar days from the date of award. Updated personnel rosters (to include any required documentation for security/installation requirements for personnel) shall be submitted as changes occur and in sufficient lead time to allow/enable the Government to conduct required clearances prior to a new employee commencing with services. Proposed employees who have not been cleared up and listed on the approved personnel roster shall not be allowed to work anywhere within VASLC.

1.5.17 Task - Housekeeping Porter (Hospital): Hospital housekeeping porters play a crucial role in maintaining the cleanliness and hygiene of hospital facilities.

1.5.18 Standard - Cleaning and sanitizing, bathrooms, waiting areas, and other designated areas. Dusting, sweeping, mopping, and vacuuming floors in hallways, and common areas. Collecting and disposing of trash and medical waste properly, following infection control guidelines.

1.5.19 Standard - Restocking supplies such as towels, toilet paper, soap, and other essential items in public areas. Maintaining inventory of cleaning supplies and notifying the supervisor when items need to be replenished.

1.5.20 Standard - Reporting any maintenance issues or repairs needed to the appropriate department for prompt resolution. Follow standard procedures for handling and disposing of biohazardous materials and adhering to infection control practices.

1.5.21 Standard - Assisting with the setup and breakdown of special events or meetings as required. Adhering to all safety precautions and protocols while performing duties to ensure the well-being of patients, staff, and visitors.

1.5.22 Task - The contractor will provide coverage of main hospital areas (bldg. 1 and 14) for two (2) different shifts to the VA Medical Center in Salt Lake City on Monday, Tuesday, Wednesday, Thursday, and Friday. 1st shift 5:00 pm to 1:30 am, 2nd shift 1:00 am to 9:30 am in accordance with the Performance Work Statement.

1.5.22 Standard - The Contractor shall provide all labor, supervision, management support, to facilitate unit cleaning.

1.6.TRAINING:
1.6.1.Task: The Contractor shall ensure that all housekeeping employees attend the Contractor's monthly in-service training sessions on mandatory contract and VASLC required topics.
1.6.2.Standard: The Contractor shall provide immediately for new employees and monthly thereafter annual refresher training for all employees covering all topics and areas in the orientation training program. Every twelve-month period, safety and infection prevention and control classes shall be provided. The Contractor shall provide training for the purpose of updating and correcting deficiencies in individual performance found because of the Contractor's Quality Control Program (QCP), the Government's Performance Assessment Plan (PAP), or supervisory review. The Contractor shall provide training schedules to the COR and shall ensure that all personnel attends. Within 30 days after contract award, the Government with a copy of your company's QCP.
1.6.3.Standard: The Contractor shall provide initial orientation and continuing education documentation or certificates for all employees (including purpose of training) when requested by the COR. Records shall be kept by the Contractor, available for review at the request of the Government and a copy given to the COR for the contract records. Records shall indicate by name who attended, date and time, a brief overview of the subject, instructor's name and all attendees shall "sign in." A copy of employee training files shall be consistent with the training program required for all VASLC staff members and maintained.
1.6.4.Standard: Orientation training covering the topics below shall be started and completed within 2 weeks prior to start of work for all employees who have never received orientation training in hospital/healthcare housekeeping. Employee(s) not previously trained in healthcare housekeeping, including infection prevention and control may not be assigned to work until this training requirement is completed. Employees who have had previous healthcare training are required to complete retraining and/or specified training for VASLC within 2 weeks after starting work. Orientation training shall include but is not limited to the following topics:
1.6.5.Standard: A general orientation of basic bacteriological concepts, including the basics of how disease is caused, transmitted, and how it can be prevented, reduced, or contained through proper hand hygiene and hospital housekeeping methods.
1.6.6.Standard: An initial orientation as required by Occupational Safety and Health Administration (OSHA) and TJC and at least annually thereafter, on topics concerning Occupational Exposure to Blood Borne Pathogens.
1.6.7.Standard: Complete instruction on the proper selection, measurement, use of disinfectants, cleaning supplies, procedures for replenishing supplies and use of sustainable green cleaning products.
1.6.8.Standard: Instructions on floor-care application and maintenance to include stripping, waxing, buffing, spray buffing, dust and damp mopping.
1.6.9.Standard: Proper care and preventative maintenance of Government furnished equipment and supplies.
1.6.10.Standard: The Contractor shall be familiar with local fire prevention, safety, TJC criteria and applicable hospital and local base regulations and policies (including disaster plan and base access).
1.6.11.Standard: Familiarization with all Contractor printed technical, Procedural Manual and Hazardous Communication Exposure Control Programs. Hazardous Communication Program shall include procedures for handling all hazardous housekeeping chemicals, storage of those chemicals and procedures for accidental exposure or ingestion (Safety Data Sheets) and use of Government forms for recording and upkeep of the program.
1.6.12.Standard: Role of housekeeping in the healthcare facility, its effect on the health and well-being of patients and proper attitude towards patients, visitors, and staff members and techniques for cleaning all "Type Service" areas.
1.6.13.Standard: Contractor shall provide 30 days notification prior to training to allow proper scheduling. VASLC staff members selected by the COR will at times be sent to observe the Contractor's training program for adequacy.
1.7.CONTINUING EDUCATION: The Housekeeping Supervisor shall attend at least one contract related to continuing education course yearly prior to assignment anniversary date in one or more of the following areas: (a) recent developments in infection prevention and control, (b) hospital housekeeping supplies and/or equipment, and (c) procedures and techniques. Upon completion of the course the certification shall be presented to the COR and included in the personnel portion of the contract file.
1.7.1.Standard- The contractor shall provide initial orientation and continuing education training in environmental services and VASLC contract requirements for all employees.
1.7.2.Standard: The Contractor shall provide initial orientation and continuing education documentation/certificates for employees (including purpose of training) if requested by the COR.
1.8.UNIFORMS: The Contractor shall provide all housekeeping employees, uniforms (shirts and pants) of distinctive color and design distinguishable from all other VASLC departmental uniforms.
1.8.1.Standard: Contractor employees shall wear a clean uniform each day and comply with general hygiene requirements commensurate with working in a hospital environment. Loose ties, loose shirts, loose ends, loose fitting belts and hats, shall not be permitted as part of the uniform. Sandals or open-toed shoes are prohibited. Artificial nails shall not be permitted per Infection Prevention and Control Standards.
1.8.2.Standard: All the Contractor's employees shall wear Government provided surgical scrubs when working in Type I Service areas where scrubs are required.
1.8.3.Standard: The Contractor shall provide any Personal Protective Equipment/Clothing (PPE) and devices (i.e., rubber aprons, safety gloves, safety shoes, outdoor gear, etc.) at its expense. The Contractor is responsible for performing medical and respirator fit testing in accordance with 29 Code of Federal Regulation (CFR) 1910.134).

Respiratory Protection involving the N95. This testing is for the N95 respirator and is provided by the Government to all Contractor employees required to wear them where exposure to Tuberculosis is possible. Contractor employees working in the Regulated Medical Waste area shall wear fluid resistant disposable coveralls. Coveralls should be removed prior to leaving this area when work is completed or upon being used in another area. Appropriate PPE will be required for Type III Service terminal cleaning.

1.8.4. Standard: All housekeeping personnel shall display a VASLC identification badge as part of their uniform. This identification badge shall be always worn and be clearly visible.

This ID remains the property of the Government and shall be returned upon contract completion or at the time an employee terminates employment. It is the Contractor's responsibility to ensure that all Government badges are returned to COR within 24 hours of termination. The government reserves the right to confiscate the identification badge whenever the need/situation arises. The COR and EH shall discuss the need to confiscate the badge prior to confiscating the identification badge.

1.9.HEALTH REQUIREMENTS: The Contractor shall be responsible for all required pre-employment physical examinations at no additional cost to the Government unless otherwise approved by the Contracting Officer.
1.9.1.Standard: The Contractor shall provide renewal certificates, detailing compliance with health requirements, annually or as requested to the COR for each employee.
1.9.2.Standard: Contractor employees who have accidental percutaneous (needle stick), ocular or mucous membrane exposure to blood or other body fluids, or other injuries while working in VASLC facilities, will report all injuries to the COR. Contractor will be responsible for all medical treatment and costs.
1.9.3.Standard: Contractor personnel (assigned to work in patient treatment areas) who have been absent due to a potentially transmittable illness or for a period of 3 or more duty days shall present a statement from a physician that they are free from illness or disease before being allowed to return to duty. A copy of the physician's statement shall be submitted to the COR upon the employee's return.
1.9.4.Standard: The Government reserves the right to require examinations or reexaminations of any employee as determined necessary by the COR. Any such examinations shall be performed by the Government at Government expense.
1.9.5.Standard: Contractor staff shall have received required immunizations (screenings and vaccines) prior to beginning work at the VASLC, and thereafter at the frequency stipulated in the table below. The Contractor shall forward current immunization records for all employees within 5 calendar days of any Government request. Contractor shall forward Tuberculosis and Hepatitis B test results to the COR before start of work and within five calendar days of request by the COR.

1.9.6. Standard: Special immunizations and biological testing may be administered by the VASLC at Government expense in the event of unusual risk situations. If required by the VASLC Infection Control Committee (ICC), the Government will provide screening of Contractor staff. This will include Tuberculosis and Hepatitis-B test results.

1.10.PERSONNEL MISCELLANEOUS:
1.10.1.Standard: Contractor employees shall be able to read, write, speak, and understand English to effectively communicate with Government personnel.
1.10.2.Standard: The Contractor shall turn in total monthly hours worked on the first day of every month for the previous month to the COR.
1.11.SECURITY REQUIREMENTS:
1.11.1.All contractor employees are subject to the same level of investigation as VA employees who have access to VA sensitive information or access to VA facilities. The background investigation includes the following requirements: 1) Completed documentation 2) Fingerprints 3) Completion of OPM’s e-QIP Questionnaire. The Contractor is required to fulfill all security requirements. The Contractor, upon completion of fingerprinting, and an initial suitability determination, may be authorized tentative access to start the performance period of the contract, but only on condition of completion of all security requirements. This requirement is applicable to all subcontractor personnel requiring the same access.
1.11.2.Standard - Background Checks: Employee background checks, to include checks of areas of residence for the last seven years, shall be accomplished by VASLC for all employees. Contractor shall submit an initial personnel roster to the COR within 30 calendar days of award for all personnel (name, date of birth, and social security number) proposed to provide housekeeping services. Background checks shall first be submitted to the COR and the COR will submit it to the appointed Security reviewers for initial acceptance. Updated rosters shall be forwarded to COR as changes in personnel occur. Contractor employees shall not be permitted to work at the VASLC until the security check has been initiated and accepted. Any subsequent reports from security checks prohibiting the Contractor's personnel from providing services shall be brought to the immediate attention of the COR. The Contractor shall immediately remove the employee within 24 hours of COR notification. Removal of the employee shall not relieve the Contractor of its obligations to perform under the contract.
1.11.3.Standard: The Government reserves the right to require removal from the jobsite any Contractor employee who endangers persons or property (e.g., under the influence of drugs, alcohol, etc.) and whose continued presence at the VASLC is inconsistent with the interest of facility security. Further, the Government reserves the right to refuse or permit entry on the installation of any Contractor employee who is wrongfully appropriating and/or disposing of Government property. In such cases, the Contracting Officer or his designated representative will advise the Contractor of reason for requesting an employee's removal or withdrawing his permit to enter the facility. The Government reserves the right to confiscate any contractor employee's ID badge. The contractor shall forward the terminated employee's ID badge to the COR within 24 hours of any employee's removal.

Removal for such cause does not relieve the Contractor of the requirement to provide sufficient personnel to execute requirements under the contract.

1.11.4.Standard: Contractor personnel shall observe and comply with all rules, policies, and regulations prescribed by this installation and other federal authorities concerning eating/smoking areas, fire, safety, sanitation, security and possession of firearms or other lethal weapons
1.11.5.Standard: Contractor employees shall immediately depart the facility upon completion of their assigned shift.
1.11.6.Standard: The Contractor shall not employ aliens who do not have a valid U.S. Immigration Form 1-151, Alien Registration Card, in the performance of the work under this contract in violation of the Immigration Laws of the United States. The Contractor shall require such documentary evidence of his/her employees and take such other action as is reasonably necessary to ensure compliance with this provision. Once award is made, all employees of the Contractor will have the same level of investigation as VA employees who have access to VA sensitive information or access to VA facilities. The background investigation includes the following requirements: 1) Completed documentation 2) Fingerprints 3) Completion of OPM’s e-QIP Questionnaire. The Contractor is required to fulfill all the security requirements.
1.11.7.Standard - Motor Vehicle Registration: All motor vehicles operated on this installation by the Contractor and its employees shall be registered in accordance with State of Utah registration procedures.
1.11.8.Standard: All Contractor employees/representatives entering the Government installation shall abide by all security regulations for the installation which may be in effect during the contract period and shall be subject to such checks as may be deemed necessary to ensure that no violations occur. No employee will be permitted on the installation when such checks reveal that his/her presence would be detrimental to the security of the installation.
1.11.9.Standard: The Contractor shall obtain all local, State and Federal Government licenses, passes and permits necessary prior to commencement of services.

1.11.10. Standard - Meetings and Committee Membership: The COR may require the Housekeeping Supervisor to attend scheduled and unscheduled meetings as required, at no additional cost to the Government.

1.12.BUILDING SECURITY:
1.12.1.Standard — Keys: The Contractor shall be provided key/FOBS (and/or access cards) or allowed access to all buildings and rooms requiring cleaning. The Government may choose to accompany Contractor personnel when in certain secured areas. Keys provided to the Contractor shall not be duplicated or removed from the premises. All keys/FOBS not issued for the day shall be retained in a designated locked metal key box. If a master key/ FOB is lost or duplicated by a contractor’s employee, all locks and keys to that system will be replaced by the Government. The Contractor shall reimburse the Government for the total costs involved. The Contractor's Housekeeping Supervisor shall sign for all keys/ FOBS issued by the VASLC Facility Manager. The Contractor shall maintain all record keeping and maintenance in accordance with security and key control regulations as set forth in applicable VA regulations.
1.12.2.Standard: The Contractor shall be liable for any unauthorized use of keys/ FOBS, or negligent employee conduct in work areas (work area is defined in PWS Section 1.4, Place of Performance) within that directly results in damage, loss, or theft of Government property.
1.12.3.Standard: The Contractor shall secure all work areas after completion of work. The COR shall be immediately notified of any security issue.
1.13.SAFETY:
1.13.1.Standard: The Contractor shall exercise all standard safety precautions (e.g., warning signs, interruption of work, phasing of work, etc.) to allow safe passage of patients, visitors, personnel, equipment, or carts in work areas.
1.13.2.Standard - Accident Reports: The Contractor shall submit accident reports in accordance with the provisions of VASLC policy. A copy of all accidents shall be furnished within 3 working days to the COR who, in turn, will forward it to the VASLC Safety Officer.

1.13.3 Handling/Cleaning of Government Furniture/Equipment.

1.13.4.Standard: The Contractor shall not unplug Government equipment that is plugged in for recharging.
1.13.5.Standard: The Contractor shall move furniture and equipment as required to execute housekeeping duties. Exceptions to this standard are modular furniture assemblies and extremely heavy equipment. If the Contractor feels that they might cause damage to furniture or equipment, the Contractor shall contact the COR and explain the circumstances before moving furniture or equipment.
1.13.6.Standard: The Contractor shall be liable for damages incurred while moving or handling equipment/furniture including damage to any part of the building structure if negligence is implicated. Furthermore, the Contractor shall be liable for damage to Governments property or work areas caused as a direct result of Contractor negligence, equipment, or supplies. Contractor is to notify COR of damage in writing within 24 hours.

NOTE: The Contractor is advised that the Government, not the Contractor, is responsible for moving specific computerized medical, electrical, and/or electronic equipment.

1.14.DEFINITIONS: The following terms used throughout this PWS shall have the following meanings:
1.14.1.Contractor: A supplier or vendor having a contract to provide specific supplies or service to the Government. The term used in this contract refers to the prime.
1.14.2.Contracting Officer: A person with authority to enter, administer, and or terminate contracts, and make related determinations and findings on behalf of the Government. Note: The only individual who can legally bind the Government.
1.14.3.Contracting Officer’s Representative: An employee of the U.S. Government appointed by the Contracting Officer to administer the contract. Such appointment shall be in writing and shall state the scope of authority and limitations. This individual has authority to provide technical direction to the Contractor if that direction is within the scope of the contract, does not constitute a change, and has no funding implications. This individual does NOT have authority to change the terms and conditions of the contract.
1.14.4.Deliverable: Anything that can be physically delivered but may include non-physical things such as meeting minutes.

1.14.5 Performance Requirements Summary (PRS): A document that captures in table form the statements, standards for each statement.

1.14.6.Acceptable Quality Level (AQL): is an acceptable performance variation to standards established in the PWS for each task statement.
1.14.7.Quality Insurance: The Government procedures to verify that services being performed by the Contractor are performed according to acceptable standards and a planned and systematic pattern of all actions necessary to provide confidence that adequate technical requirements are established; products and services conform to technical requirements; and satisfactory performance is the result.
1.14.8.Quality Control: All necessary measures taken by the Contractor to ensure that the quality of a product or service shall meet contract requirements.
1.14.9.Quality Assurance Surveillance Plan (QASP): An organized written document specifying the surveillance methodology to be used for surveillance of contractor performance.

1.14.10. Advisory Documents: Directives, which the Contractor may use for information and guidance, and which are not binding for compliance.

1.14.11. Blood borne Pathogens Standards: The Occupational Safety and Health Administration (OSHA) standard to eliminate or minimize occupational exposure to Hepatitis B Virus (HBV), Human Immunodeficiency Virus (HBV), and other blood borne pathogens.

1.14.12. Steadfast Policing: The clean-up of papers and any other debris, removing spills, vomitus, spots and marks from walls, loading docks, walkways, entrances, parking lots, interior planters, floors, carpets, furniture, exterior trash receptacles and cigarette ash trays/receptacles.

1.14.13. Disinfection Cleaning: Falls between the processes of physical cleaning and sterilization as the elimination of disease-producing microorganisms, but not spores, from inanimate objects via pasteurization or liquid chemicals. Create a clean, safe, attractive environment for patients, staff, and visitors.

1.14.14. Facility Defects: Flaws in the physical plant caused by age, breakdown, damage, or other causes. These may be cosmetic or causing further damage to personnel. The Government will identify defects as routine, or emergency based upon urgency of repair.

1.14.15. Hard Surface Floors: Floor surfaces that do not require application or removal of floor finish or polishing. Examples include grouted tile floors in latrines and utility rooms.

1.14.16. Infection Control Committee (ICC): A designated group of hospital staff responsible for monitoring the total infection control program within the medical facility. VA and TJC consider housekeeping an integral part of the total infection control program.

1.14.17. Housekeeping Services: The hospital housekeeping department's primary function is to develop and maintain a sanitary, safe, and esthetically pleasing environment that will execute the primary mission of patient care. Contractor management, tasks, and standards shall be developed to maintain a health care environment that is in strict compliance with state, federal, local, and VASLC guidelines and regulations. The Contractor shall, in addition to all work areas identified under contract, execute any and all tasks required to execute required services under contract. This includes all areas in a room from the floor to the ceiling, including lights.

1.14.18. Internal Emergency Clean Up: Emergency response by Contractor employees may be required (over and above routinely scheduled work) to provide for patients and staff welfare and safety of the building because of mass casualty situations or natural disasters. Examples include facility defects, fire, mass illness or an injury situation. The facility shall be maintained, as closely as possible, in a hygienically clean condition during emergency situations.

1.14.19. The Joint Commission: A national organization dedicated to improving the care, safety, and treatment of patients in health care facilities, and publishers of the Joint Commission on Accreditation Manuals.

1.14.20. Key Personnel: Key personnel consist of the Housekeeper supervisor and Shift Leaders. Mandatory Documents: Directives with which the Contractor is obligated to comply.

1.14.21. Non-Emergency Clean Up: The clean-up of paper, water, and any other debris, removing spills, vomitus, spots, and marks from walls, docks, entrances, parking lots, floors, carpets, and furniture. The Contractor shall provide emergency pickup and removal of spillage and other similar accidents throughout the facility during regular business hours, within 10 minutes after receiving notification of each incident. (This does not include hazardous spills).

1.14.22. Healthcare acquired Infection: An infection acquired by patients and others during hospitalization, with diagnosis confirmed by clinical or laboratory evidence. Great danger arises with hospital infections because of the weakened condition of patients. The most effective method of defense is a clean, infection-free environment. Effective environmental sanitation is required to lessen hazards to the extent that healthcare acquired infections may be consequence of exposure to contaminated air, dust, furnishings, equipment, and other fomites. Frequent and thorough cleaning of hospital interiors is necessary to reduce the number of pathogens. The main purpose is to physically remove microorganisms from the various fomites that might, in turn, transmit them to patients.

1.14.23. Patient Discharge Cleaning: This service encompasses total cleaning within the patient cubical area (in multi-bedrooms) or the entire room (for single-bed rooms) whenever the patient's stay has ended or when the patient has been transferred.

1.14.24. Performance Assessment Plan (PAP): A Plan, which provides the COR a systematic method to monitor Contract performance.

1.14.25. Quality: The Standard of Cleanliness of an Area: This is measured by the Contractor's compliance with Government mandated standards or service outputs.

1.14.26. Steadfast Policing: The clean-up of papers and any other debris, removing spills, vomitus, spots and marks from walls, loading docks, walkways, entrances, parking lots, interior planters, floors, carpets, furniture, exterior trash receptacles and cigarette ash trays/receptacles.

1.14.27. Regulated Medical Waste (RMW): Regulated Medical Waste is any waste from patients in strict or respiratory isolation or with wound and skin precautions; wastes from the laboratory; and other wastes, such as surgical waste, needles, syringes, and sharps, classified as infectious. Pathological waste is any waste, which includes anatomical parts of humans and animals, excluding human corpses and animal carcasses.

1.14.28. Total Disinfection Cleaning: The highest degree or level of disinfection through cleaning of all environmental surfaces (The interruption of the chain of infection.) Total disinfection cleaning is applicable to service Types I and III. The quality standard for performance of total disinfection cleaning is to significantly reduce the potential that large numbers of microorganisms on environmental surfaces will encounter patients. It requires that a specific task be performed in a systematic manner on a daily basis.

1.14.29. Surgical Areas: Refers to all rooms/areas in the VASLC where surgical or invasive diagnostic procedures are performed, to include operating rooms, endoscopy rooms, and recovery areas.

1.14.30. Infectious Disease Management – to prevent and minimize exposure to contagious disease and disrupting disease transmission the following are recommended by the CDC and VHA for all workers in healthcare facilities.

1.15. FACILITY DEFECT REPORTS: The Contractor shall report noted facility defects (dripping faucet, loose window, graffiti, etc.) in writing within the same duty day of discovery. Emergency facility defects (such as broken water pipes, etc.) as soon as possible after discovery to facilitate repairs and shall be reported in writing by the end of the work shift.

1.16.GOVERNMENT-FURNISHED PROPERTY AND SERVICES: The Government shall provide for certain Contractor requirements as set forth herein.
1.16.1.Regulations and Other Directives: The Government shall provide, or issue required VASLC, VA, and Government items prior to start of contract performance.
1.16.2.Facilities: The Government will provide adequate space for the Contractor to use as an office (or cubicle), supply, and equipment storage, including housekeeper's closets. All space designated for Contractor use shall be kept clean by the Contractor and will be included in random inspections. Corrections for deficiencies shall be completed within 24 hours. At any time during the contract this space may be withdrawn from the Contractor due to intended use by the Government. The contract does not guarantee that space will always be available. The Government will furnish the following related services:
1.16.3.The Government will provide local phone service.
1.16.4.Surgical Garments: Contractor staff assigned to perform housekeeping service in Type I areas shall be required to wear surgical garments supplied by the Government which consist of the following items: gown, gloves, mask, shoe covers, and head cover. Isolation garments consisting of disposable isolation gown, gloves, and mask shall be worn in accordance with the isolation room door instruction card, and the isolation precautions system used by the facility. Such garments shall remain the property of the Government and shall not be removed from the premises. Such uniforms shall be worn only in areas designated by the VASLC. Employees shall change back to their Contractor provided uniforms prior to leaving these designated areas.
1.16.5.Keys/ FOBS: The Contractor will take receipt of Government issued keys from the VASLC Facility Manager.
1.16.6.Consumable Supplies: The Government will not provide consumable supplies except for the following:
-Trash Liners
-Red trash liners for Regulated Medical Waste.
-All Sharps Containers.
-All tape and container materials for RMW transportation.
-VA approved hand hygiene products (soaps, foams, sanitizer liquids, etc.)
-VA approved Cleaner and Disinfectant Wipes such as CAVIWIPES 10X12
-All paper products
-All EPA registered disinfectants or chemicals
-Mask Face Procedure Pleated

1.16.7 Computer: The Government will provide one computer for the Housekeeping Supervisor who will be required to train and maintain the necessary clearances for use of a Government Computer along with compliance with the Rules of Behavior. The computer will be used to provide updates and communications between the Facility staff and the Contractor.

1.16.8 Government Equipment: Contractor is responsible for furnishing all supplies and equipment necessary to support performance of this contract. Government will supply toilet tissue, paper towels, soap dispensers including hands free dispensing paper/soap products, liquid hand soap, cake soap, plastic trash bags suitable for general trash, partition trash liners for restrooms, toilet seat sanitary covers, feminine hygiene products (to go into Government-provided dispensers), floor wax, cleaning and disinfecting chemicals, deodorant blocks, urinal screens, housekeeping carts, recycle supplies, personal protective equipment, and pagers/cell phones.

1.17.CONTRACTOR REQUIREMENTS:
1.17.1.Task — General: The Contractor shall furnish and maintain all equipment, supplies, vehicles (other than those specified as Government-furnished) and all other items necessary to perform all services required.
1.17.2.Standard: Equipment, supplies, vehicles, and all other items shall comply with commercial industry, Federal, and State occupational safety, health, and fire regulations/standards.
1.17.3.Standard: The Contractor shall not use any supplies or equipment prior to receiving the approval of the COR. New supplies and equipment shall require product literature and may require samples. The use of aerosol sprays is prohibited in patient treatment areas.
1.17.4.Standard: The Contractor shall keep all containers of cleaning chemicals and similar products conspicuously marked with a label to identify contents and instructions on remedies for accidental poisoning; all other labels must be removed or defaced. Materials bearing a Department of Transportation (DOT) and red label (flammable) labels shall not be used. The Contractor shall obtain Safety Data Sheets, (SDS), OSHA Form 20, for all chemical products used and provide a copy to the COR. The COR will route the new SDS documents through Infection Control, Safety, and Facilities Management for concurrence prior to granting approval for use to the Contractor. New products shall not be used until approved by the COR. The Contractor shall keep copies of all Safety Data Sheets (SDS) in the Housekeeping Office and in all housekeeping closets.
1.17.5.Standard: Contractor's equipment shall be compatible with existing sources of Government- furnished electrical power and follow UL and National Fire Protection Association (NFPA) requirements.
1.17.6.Standard: The Contractor shall store equipment, materials, and supplies in the designated areas when not in use. The contractor will be provided with lockable storage space for storing its equipment and supplies. Stored trash containers and receptacles shall always be visibly clean.
1.17.7.Standard: The Contractor shall dedicate, clean and disinfect as appropriate, and permanently mark equipment, materials, and supplies to be utilized in total disinfection work areas.

NOTE 1: The Contractor shall use pagers and/or cellular phones and shall be of sufficient quantity to accomplish the housekeeping mission throughout VASLC.

NOTE 2: The Contractor shall maintain an itemized list of all supplies/chemical inventory/equipment and shall provide an updated list to the COR annually for approval.

1.17.8.The Contractor shall be responsible for furnishing all housekeeping uniforms except for those provided by the Government. Contractor is responsible for furnishing all supplies and equipment necessary to support performance of this contract. Government will supply toilet tissue, paper towels, soap dispensers including hands free dispensing paper/soap products, liquid hand soap, cake soap, plastic trash bags suitable for general trash, partition trash liners for restrooms, toilet seat sanitary covers, feminine hygiene products (to go into Government-provided dispensers), floor wax, cleaning and disinfecting chemicals, deodorant blocks, urinal screens, housekeeping carts, recycle supplies, personal protective equipment, and pagers/cell phones.
