About this file

This document contains questions and answers related to a renovation project for a dental clinic at the Sheridan Veterans Affairs Medical Center in Wyoming. The project scope involves renovating Building 9 to create a new dental clinic and will include removing and reinstalling ceilings, installing new flooring after moisture testing, and reinstalling owner-furnished dental chairs and x-ray machines. Clarification is provided that the contractor must install a ceiling-mounted lift but an exterior lift may be acceptable, and that drawings provide enough information for structural elements connecting to an existing masonry chimney. Piping with asbestos insulation will be removed if encountered during demolition.

RFI Questions VA Comments in Red AE Comments in Blue Questions 1

1) In Sheet "QH1.00 Equipment Plan - Level 2" Item number 18 lists a Ceiling Mounted Lift system to be Contractor Furnished and Installed. However, in Sheet "E100 Power Plan" Keynote #5 refers to the lift as OF/CI. Could you clarify if the lift system is part of the contractor’s scope and bid submission? Change reference on the electrical sheet to CF/CI

2) If so, may equal or better substitute products matching the listed supplier (Horcher Unilift) in specification be accepted? Yes

3) Do we need to install a moisture remediation system under sheet vinyl?

Per 3.1 G in the specs.

Perform flooring manufacturer’s recommended bond, substrate moisture content, and pH tests.

4) After the leveling is complete, will there need to be moisture and PH testing done?

Per 3.1 G in the specs.

Perform flooring manufacturer’s recommended bond, substrate moisture content, and pH tests.

5) Specification table of contents indicates the following sections that we are not finding in the documents we received:

a) 02 82 13.13 – Glovebag Asbestos Abatement This section will be added

b) Division 05 – MASONRY – Should be STEEL, Will there a Division 4 MASONRY specification? No, Delete from Table of Contents

d) Division 10 – Specialties – Lockers? This section will be added

6) Will a copy of the site visit sign in sheet be provided? Yes

7) Will it be acceptable to have a material lift attached to exterior of building as elevator cannot be used for construction. Yes, coordinate with the COR on location and anchoring.

Questions 2 Will the VA provide installation drawings for the Owner Furnished Contractor Installed Dental Chairs shown on Equipment Plan QH1.00. The Manufacturer (A-Dec) shall provide installation drawings upon request, for the existing Dental Chairs. Everything needed to install the dental chair is the responsibility of the contractor. Point of Contact for A-Dec systems.

Grant Austin Territory Manager, Schools & Government A-Dec Phone 800.547.1883 Mobile 503.537.4412 Fax 503.537.2702 a-dec.com

8) Does the contractor need to use manufacturer certified dental chair installers to ensure the manufacturer will honor the chair warranty? The manufacturer will cover manufacture defects related to the products. The contractor will need to remove the dental chair from the existing clinic and reinstall at the new clinic working with the COR on timing and phasing. After installation, the chairs operations and connections will need to be certified by the contractor. On Sheet QH1.00 Equipment Schedule, “Item 1” change “Furnish / Install” to “Owner Furnished, Contractor Install”

9) Please confirm that the contractor is to provide and install the GENDEX INTRAORAL X-RAY SYSTEM shown on Equipment Plan QH1.00 VA: No, the x-ray machines will be purchased through the facility (these are medical equipment). On Sheet QH1.00 Equipment Schedule, Item 4 change “Furnish / Install” to “Owner Furnished, Contractor Install”

10) The Equipment Plan QH1.00 shows a QTY of 7 EA GENDEX INTRAORAL X-RAY SYSTEMS. Only 5 are shown on the Equipment Floor Plan. There is only five (5) x-ray machines. The x-ray machines will be purchased through the facility (these are medical equipment). The contractor will need to install the provided x-ray machines in the new clinic working with the COR on timing and phasing. After installation, the x-ray machines operations and connections will need to be certified by the contractor. There will be 5 machines total: 3 between the 6 south operatories and 1 each in room OR 209 and OR 210.

11) Please confirm that the Contractor is to provide and install the patient lift shown on Equipment Plan QH1.00 Correct, see Equipment Plan Level 2 Drawing Number QH1.00 Item Number 18.

12) Will the VA’s A/E provide the engineering for the Contractors proposed shoring plan or will a 3rd party engineer have to be hired by the contractor for design and calculations? See Sheet S0.01 General Note 1. If needed, any 3rd party engineer would be the responsibility of the contractor.

13) Will the VA provide a Fire Protection Plan for the project? Provided by the contractor, see spec 21 13 13 – 2 1.4 A

14) Elevation 10 - OR 2019 has a 01-48 call out at cabinets. Should this call out be 01-08 to match elevations 09 and 11 that indicates “CABINETRY GOVERNMENT FURNISHED, GOVERNMENT INSTALLED”. On Sheet A5.01 Change Keynotes: 01-08 to “CABINETRY CONTRACTOR FURNISHED; CONTRACTOR INSTALLED”. On Sheet A5.01 Elevation Detail 10 Change Keynote references 01-048 to 01-08

15) Sheet AD1.11 Demolition Reflected Ceiling Plan – Level 1 and 3 has keynote 02-54 “REMOVE AND REINSTALL EXISTING CEILING AS REQUIRED. REPLACE ANY DAMAGED TILES.” in several rooms. During site visit we noted that Corridor EC 100 and Mechanical E106 did not have ceilings. We did not go into Pharmacy E109, Housekeeping E108, Biomed E103 and storage rooms. Can you clarify which rooms have ceiling so we can better determine where ceiling will need to be removed for demolition or new MEP work? Sheet AD1.11 Remove note 02-54 from room Mechanical E106 and room Corridor EC100

16) Posts identified in drawings do not show elevation of footer. Is the footer on the first floor or in the basement? All columns bear on either stone or brick foundation/bearing walls. Top of said walls are assumed to be within 8”-12” or less of finished floor of Level 2

17) I beam relation to channel iron. The drawings do not show structural integrity to one another. What are they? If the area in question is referencing the steel beams around the (E) Masonry/Stone Chimney, the following should clarify:

1. Elevation of C-Channel beams around Chimney are called out on plan as 122’-8”

2. Elevation of W10x19 beam on the plan West side of chimney is 123’-9 ¾” (to be Field Verified based on existing joist framing).

3. Elevation of W10x22 beam on the plan East side of the chimney is 123’-9 3/8” (to be Field Verified based on existing joist framing).

4. Section C3 and Elevation F1 on Sheet S5.01 visually depicts locations of the beams and their location in relation to one another.

18) Connections of channel iron to the chimney location. As a chimney cannot be used as a structural support, as shown in Detail C3. For placement of bolts to chimney, will there need to be a post to support the weight of the channel, in that area? The existing Masonry/CMU chimney/chase that is referenced on the drawings is an abandoned structural element. If the worry was about installing framing members and through bolts to an active chimney, that is not the case we have here. Thus, we still intend to use this existing stone/masonry structure as a load-bearing element and no new columns will be required.

19) Note that the details on Drawing S 5.01, contain enough information as it applies, in the field. Revised interpretation of the question above: “Does the details on Drawing S 5.01 contain enough information as it applies in the field?” Answer: Yes

20) Drawing A1.01 Sheet Specific Note #5 Install blow in insulation within the second floor joist cavity throughout the project area. There is nothing else mentioned in any of the key notes or anywhere else on the plans or in the specs. Is it the intent that after the flooring is removed and before it is levelled to drill holes in the sub floor throughout the project area and spray insulation between the joists? If so, are the floor joists boxed in with plywood so that the insulation would have something to adhere to. This was a note that came over from the 2012 Construction Documents and I assume it was done for sound dampening. Our understanding is that there is a plaster layer across the bottom of the floor joists. The insulation could be blown in from below and should be held in by the plaster ceiling and the joists on the side.

21) Note 02-52 Drawing AD1.10 States:

REMOVE ACT CEILING SOFFIT AND FRAMING COMPLETELY LOCATED ABOVE EXISTING ACT CEILING, SHOWN HATCHED. EXISTING ABANDONED PIPES WITH THERMAL INSULATION CONTAINING ASBESTOS MATERIALS, WHERE OCCURRING, SHALL BE REMOVED AND DISPOSED OF AS SPECIFIED IN SECTION 02 82 13, ASBESTOS REMOVAL AND DISPOSAL.

Please Provide the LF and Size of abandoned pipes to have asbestos insulation removed.

If Qty’s are unknown please provide an allowance for contractors to carry.

With projects in the older buildings in Sheridan notes are placed on all drawings as to the possibility of asbestos. When it occurs within an active project, we then evaluate needed methods and a change modification is created accordingly.
