About this file

This is an Amendment to Solicitation/Modification of Contract (Standard Form 30) issued by the Department of Veterans Affairs, Network Contracting Office 17, San Antonio, Texas, for solicitation number 36C25726Q0888, effective September 23, 2026. The amendment provides answers to 29 technical questions submitted by offerors regarding a medical gas inspection, testing, and certification contract at VA Medical Center (VAMC) Dallas.

The Government clarified key contract scope items and operational requirements. Medical air compressor systems total 8 units across multiple building locations, manufactured by Chemetron, Powerex, Atlas Corp, and Beacon, with specified model and serial numbers provided. Medical-surgical vacuum pump systems total 7 units from manufacturers including Chemetron, Quincy, and Beacon, with detailed pump model and manufacturer information. The contract covers 4,381 patient terminal outlets/inlets (DISS disconnects are component parts of terminal assemblies, not separate items), 473 zone valves, 133 master alarm panels, and 152 area alarm panels. Dryer packages by source unit and dew point/CO monitors are specified by manufacturer and model. The Government confirmed that CLIN 0001 covers scheduled annual inspections and preventive maintenance on an annual cycle, while CLIN 0003 (NTE, to be established by Government) covers unscheduled out-of-cycle testing. Offerors must not propose their own pricing for NTE CLINs. Zone-valve particulate sampling is representative sampling of each gas system pipeline (33 contaminant test sites total), not per-valve testing of all 514 valves. The 30-day baseline inspection requirement is a Performance Work Statement determination. The incumbent contractor is identified as Evergreen Medical Services (Indianapolis, IN), with the most recent annual inspection dated October 13-17, 2025, achieving an overall health rating of 93.1%. Emergency responders should be credentialed as ASSE 6040; follow-on verification and repair work should be charged to NTE CLINs. Monthly progress payments are not permitted; invoices for fixed annual work are submitted when services occur. Past-performance references must be from the prime contractor, not key personnel or subcontractors.

7. Prior-Year Reports – copies of the most recent annual medical gas inspection/certification reports, preventive maintenance reports, deficiency reports, and equipment condition/asset reports, if available.

The attached 2025 annual report covers certification, condition, and deficiencies.

Deficiency summary in item 12.

8. Out-of-cycle services under CLIN 0001 and CLIN 0003 The description of CLIN 0001 includes “out-of-cycle (unscheduled) inspection, testing and certification,” while CLIN 0003 separately establishes an NTE line item for the same type of work. Please clarify whether the fixed price for CLIN 0001 should include any out-of-cycle services or whether all such work will be performed exclusively under CLIN 0003. CLIN 0001 is for scheduled inspections. CLIN 0003 is for unscheduled out of cycle inspections.

9. Pricing of NTE CLINs CLINs 0002 and 0003 state that the Government will enter a Not-to-Exceed amount upon award. Should offerors leave the unit-price and amount fields for these CLINs blank, enter “NTE—To Be Established by the Government,” or propose their own NTE amounts? Unless the offeror is the Government, then do NOT put pricing within these CLINs. Offerors are NOT the Government so they should NOT propose their own NTE amounts.

10. Anticipated NTE amounts and historical usage If available, please provide the anticipated annual NTE amounts for correction of deficiencies and out-of-cycle testing. Please also provide the approximate number and dollar value of routine repairs, emergency repairs and out-of-cycle certifications ordered during the most recent contract year.

Unless the offeror is the Government, then do NOT put pricing within these CLINs.

Offerors are NOT the Government so they should NOT propose their own NTE amounts.

11. Complete equipment inventory PWS Section 7 identifies most equipment quantities but lists medical-surgical vacuum pumps as “As installed (field-verify).” Please provide the number, manufacturer, model and configuration of the installed vacuum pumps.

Answered in question 1,4,5, and 6

12. Manufacturer and model information To permit accurate pricing of preventive maintenance and standard consumables, can the Government provide an equipment inventory identifying the manufacturers, model numbers and current PM schedules for:

Answer: The manufacturers and model numbers for all source equipment are provided in the equipment tables in this amendment (medical air compressors and medical-surgical vacuum pumps). Dryer packages by source unit are as follows:

Location / Serving Dryer MFR Dryer Model BLDG 2J Mech BB-260 (MAC1), 2J North Quincy 2010327925 / QMOD00175 BLDG 2J Mech BB-260 (MAC2), 2J South Quincy Unavailable BLDG 2J South Main Mech BB-260 Atlas Corp CD65 BLDG 2 Basement MAC1 Trident Dryspell 100 BLDG 2 Basement MAC2 Trident (model unavailable) BLDG 60 Penthouse Beacon (model unavailable) BLDG 74 SCI Basement BA-120 Zander K-MT 4 MED BLDG 2J Lab Air BB-260 Zander K-MT 2 RX

Preventive maintenance is performed on an annual cycle consistent with the fixed-price CLIN. The Government is not identifying additional quarterly or semiannual PM intervals; offerors should price PM on an annual basis.

13. Annual testing quantities

Please confirm whether the contractor must inspect and test all 4,279 patient terminals and all 4,279 manufactured assemblies/DISS disconnects during each contract year, rather than testing a representative sample.

Self-answering per the doc; the report's outlet/inlet inventory counts each terminal assembly once (4,381 total lines), which indicates DISS disconnects are part of the terminal assemblies, not separate additive components.

14. Potential duplication in the documented inventory Please clarify whether the listed 4,279 DISS disconnects are separate components in addition to the 4,279 patient terminals, requiring 8,558 individual component inspections, or whether some or all DISS disconnects are part of the listed patient-terminal assemblies.

The Government's most recent annual inspection report (attached) constitutes the equipment inventory the contractor will be responsible for maintaining and inspecting, identifying manufacturers, model numbers, locations, and quantities for all in-scope components:

• Medical air compressor systems — 8 systems (see compressor table)

• Medical-surgical vacuum/WAGD systems — 7 systems (see vacuum pump table)

• Medical air dryer packages — per source unit (Quincy, Zander, Atlas Corp, Trident, Beacon; see dryer table)

• Dew point / CO monitors — per source unit (e.g., GfG Instrumentation 4021-DPX, Enmet CO-Guard)

• Manifolds for liquid containers — N2O (Bldg 2J BD-104), N2 and CO2 (Bldg 2J BD- 101)

• Cryogenic bulk oxygen system — Messer, Bulk Pad at Energy Plant Bldg 70

• Master alarm panels — 133 signals across the facility (Amico A2M, Beacon 0321-77)

• Area alarm panels — 152 signals (Allied, Amico, Beacon, Tritech)

• Zone valves — 473 total

• Outlets/inlets (patient terminals) — 4,381 total, by manufacturer/model/gas

Manufacturers and model numbers for each component are listed line-by-line in the corresponding report sections. Preventive maintenance is performed on an annual cycle consistent with the fixed-price CLIN

15. Zone-valve particulate sampling PWS Section 8.2(c) requires the contractor to collect particulate samples in each zone.

Please confirm whether sampling and laboratory analysis are required for all 514 zone valves annually. If so, please identify the required sampling procedure, laboratory standard and acceptance criteria.

Zone-valve particulate sampling – all 514 zone valves annually?

The report shows 473 total valves and only 33 contaminant test sites. Per the report's stated procedure, particulate sampling is <cite index="2-1">a representative sampling of each gas system pipeline… using a 0.45 micron filter membrane connected to an outlet of each gas</cite> — representative, not per-valve. This supports declining the all-514 interpretation.

16. Baseline inspection schedule PWS Section 8.1 requires completion of the initial comprehensive inspection and certification within 30 calendar days after award. Please confirm whether this means that all listed equipment and components—including the 4,279 patient terminals, 4,279 DISS disconnects, 514 zone valves and 274 alarm panels—must be inspected, tested and documented within that 30-day period.

30-day requirement is a PWS determination.

17. Multiple work crews Will the Government permit multiple certified inspection teams to work concurrently in different areas of the medical center to meet the 30-day baseline requirement?

Contractor means and methods

18. After-hours work PWS Section 12 requires surgical-suite testing after normal working hours. Please provide the approximate number of operating rooms, procedure rooms, recovery areas and associated components requiring after-hours testing. Please also identify the customary after-hours testing window.

Inventory shows OR 1–13/Cysto, 13 PACU recovery bays, 3 ISO recovery rooms, plus cath/EP/GI procedure rooms. Customary after-hours window is a Government answer, not in the report.

19. PM frequencies exceeding annual service PWS Section 9 requires the contractor to perform manufacturer-required preventive maintenance when the specified frequency is more frequent than annually. Please identify which equipment currently requires quarterly, semiannual or other recurring PM visits so that offerors can include those visits in their fixed annual pricing.

Annual PMs are and maintenance are perform on master control panels.

20. Emergency-response workload Please provide the approximate number of emergency service calls received during the previous 12 months. Please also confirm whether the four-hour on-site response requirement applies every day of the year, including weekends and federal holidays.

unknown

21. Emergency-response technician requirements Please confirm whether each technician responding to an emergency must personally possess ASSE 6040 or ASSE 6010 certification, as applicable, and whether an ASSE 6030 verifier must also attend every emergency repair or only perform the required follow-on verification. Responders should be credentialed as ASSE 6040.

22. Current inspection and deficiency information If available, please provide the most recent annual medical-gas certification report, preventive-maintenance report, equipment-condition report and open-deficiency list. This information would materially improve price comparability among offerors.

Provided via the attached report; deficiency summary in item

23. Incumbent contract information Please identify the incumbent contractor and previous contract or purchase-order number, if applicable, and advise whether historical task-order, repair and pricing information is available.

The 2025 report identifies the inspecting firm as Evergreen Medical Services (Indianapolis, IN).

24. Annual fixed-price invoicing Section B.1 states that invoices will be submitted monthly in arrears. Please clarify whether the fixed annual inspection and PM CLIN may be invoiced through monthly progress payments based on completed work or whether the entire annual CLIN may be invoiced only after completion and acceptance of the annual inspection, certification and PM program.

Invoices are NOT paid through progress payments. Invoice for the fixed annual inspection and PM CLIN when these services occur.

25. Repairs and follow-on verification When an authorized repair requires follow-on testing, should the repair labor be charged to the correction-of-deficiencies CLIN and the follow-on verification labor be charged separately to the out-of-cycle-testing CLIN?

Should be charged against the NTE CLIN

26. Standard PM consumables PWS Section 9 states that consumable filter elements and standard PM consumables are included in the annual fixed price. Please provide a list of the filters and other consumables historically replaced, including approximate annual quantities, manufacturer and part number, where available.

Inspections should annotate if the filters should be changed.

27. Required permits and licenses The offer-submittal instructions require applicable permits, licenses and certifications. In addition to the stated ASSE 6030, ASSE 6040 and/or ASSE 6010 personnel certifications, please identify any specific Texas contractor license, plumbing license, laboratory accreditation, company certification or local permit the Government expects offerors to submit.

ASSE 6010 – Medical Gas Systems Installer The person who physically installs the piping. This certifies competency in brazing, joining, and installing medical gas and vacuum piping systems to code. An installer works under the direction of others and does not perform the independent verification.

ASSE 6040 – Medical Gas Systems Maintenance Personnel The person who maintains, inspects, and services existing systems — the day-to-day upkeep, preventive maintenance, and routine testing of installed systems. This is the credential most relevant to the recurring inspection and PM work in your contract. In the attached report, the inspecting technicians are credentialed to this level (the report cites its techs as qualified per NFPA 99 5.1.14 to ASSE 6040 and/or 6030).

ASSE 6030 – Medical Gas Systems Verifier The independent verifier who performs the final testing and certification confirming a system is safe and code-compliant before it's placed in service, and who verifies systems after repairs or modifications. This is the highest-authority testing role and must be independent of the installer to avoid conflict of interest. A verifier confirms cross-connection testing, purity, pressure, alarms, etc., and signs off that the system meets NFPA 99.

28. Past-performance references

The solicitation requests up to three contracts completed or performed within the last three years. May an SDVOSB prime offeror rely upon and submit the recent, relevant past performance of its proposed key personnel or similarly situated SDVOSB subcontractor when the prime itself does not possess corporate past performance of similar size and scope?

No, since the ensuing contract will NOT be awarded to the subcontractor. Contracts are awarded to the Prime so it is the Prime’s past performance that needs to be submitted.

29. Proposal email subject and solicitation number The technical-question instructions identify “36C257Q0888” in the required email subject line, while the SF 1449 identifies the solicitation as 36C25726Q0888. Please confirm that solicitation number 36C25726Q0888 should be used on the quotation and all proposal documents. This question answers itself.
