3. INVOICES: Invoices shall be submitted in arrears: Upon completion, inspection and acceptance.
4. GOVERNMENT INVOICE ADDRESS: All Invoices from the contractor shall be submitted electronically in accordance with VAAR Clause 852.232-72 Electronic Submission of Payment Requests.

SCOPE OF WORK
Waco Building 8 Access Control
REQUIREMENTS
The Contractor shall provide all necessary resources, including labor, materials, equipment, and supervision, to install access control on 6ea specified doors on building 8 Waco Campus.
The contractor shall provide all personnel, services, vehicles, transportation, tools, equipment, materials, supplies, facility supervision, administration supervision and any other item(s) necessary to perform and accomplish this service. The contractor shall be familiar with all the terms, conditions, and requirements herein contained.
All work must comply with federal, state, and local regulations.
The contractor shall be CCURE 9000 trained/certified to complete desired installation.
Period of Performance (POP) = 4 Weeks
PROJECT REQUIREMENTS:
Task 1: Installation
• Purchase and install the following: iStar Ultra G2 GCM, iStar Ultra ACM, power supply, 6ea RM4E boards, 3ea ET20 readers, 3ea ET25 readers, 6ea Request to exit modules, 6ea door contacts, 6 NGstrikes electric strikes, 1000ft SecComp cabling, 250ft of 22awg plenum unshielded cable, and 3ea BT126 EA 12vdc / 7AH Batteries.
• Deliverable: Installed iStar Ultra G2 with 6ea card readers and door hardware at specified doors within building 8.
Task 2: Verification and Testing
• Verify the iStar Ultra G2 is functioning properly upon installation.
• Verify all 6 card readers and associated hardware are wired and functioning correctly.
• Deliverable: Verified iStar Ultra G2 programmed and integrated into existing infrastructure.
Task 3: Programming and Configuration
• Program six (6) card readers. 3ea with pin at stacked OIT closets and 3ea non pin in newly constructed Pain clinic.
• Ensure the six (6) doors have all required access control configurations installed
• Deliverable: 6ea Programmed and fully configured doors at building 8.
Task 4: Final System Testing and Quality Assurance
• Conduct final system testing to ensure all iStar panels, doors, and door hardware functions as intended.
• Provide a final test report and certification of proper installation.
• Deliverable: Final test report and certification.
Note: At the end of each workday, please clean up all worked in areas and make safe.
Task 5: Contractor will contact VA Contracting Officer’s Representative (COR) or Alternative COR prior to start of the work.
Task 6: The Contractor shall schedule work through the COR, to ensure that all necessary parts, materials, and tools are accounted for prior to the start of work.
Task 7: The contractor shall coordinate with VA COR no later than (NLT) seven
(7) days to the start of this service and completion. See information below:
Government Holidays:
(a) The Government hereby provides notice, and Contractor hereby acknowledges receipt that Government personnel observe the listed days as holidays:
New Year's Day January 1st Martin Luther King's Birthday Third Monday in January Washington’s Birthday Third Monday in February Memorial Day Last Monday in May Independence Day July 4th Labor Day First Monday in September Columbus Day Second Monday in October Veterans Day November 11th Thanksgiving Day Fourth Thursday in November Christmas December 25th Juneteenth June 19th
(b) In addition to the days designated as holidays, the Government observes the following days:
a. Any other day designated by Federal Statute
b. Any other day designated by Executive Order
c. Any other day designated by the President’s Proclamation
It is understood and agreed between the Government and the Contractor that observance of such days by Government personnel shall not otherwise be a reason for an additional period of performance, or entitlement of compensation. In the event the Contractor's personnel work during the holiday, they may be reimbursed by the Contractor, however, no form of holiday or other premium compensation will be reimbursed either as a direct or indirect cost, other than their normal compensation for the time worked.
INVOICES AND PAYMENT:
Submit the proper documentation and reports to receive payment on this service. See information below:
a. Payment to be made monthly, in arrears, following satisfactory performance by Contractor’s personnel and upon receipt of a properly prepared invoice.
b. Contractor shall be required to invoice through the Tungsten System at http://www.tungsten-network.com/us/en/veterans-affairs/. Additional information regarding Tungsten shall be provided upon award.
c. The contract shall have current registration in the System for Award Management (SAM) for this solicitation; URL: www.sam.gov.
d. The invoice MUST be itemized to include the following information. Any information listed below, and not provided on an invoice, will render that invoice incomplete and the invoice will be returned for immediate correction.
i. Time period being invoiced for.
ii. Description of services performed.
iii. Unit Cost billed.
iv. Extended amount due.
v. Invoice number, date, and.
vi. Contract and obligation numbers.
CONTRACTORS RESPONSIBILITIES:
• Coordinating with the COR during the shutdown and startup of the project.
• During Veeder Root installation the contractor shall provide 7-day notice prior to any electrical or fuel interruption of service.
• The contractor shall provide specification/cut sheet on all Veeder Root and probes to be installed 14 days prior to installation. Specification/cut sheet are subject to COR and subject Matter Expert (SME) approval prior to acceptance an installation.
• Ensure that all areas affected by repairs described above are protected from damage and maintained in a safe orderly condition. Additionally, areas adjacent to work should be protected from damage. Should damage to adjacent areas occur it shall be the contractor’s sole responsibility to repair/replace damaged items at their own expense.
• With in 7days of notice to procced the contractor shall provide a written schedule for Fuel Polish service, equipment to be used and list of authorize and trained personnel providing this service. It is the sole responsibility of the contractor to ensure this schedule is uninterrupted during the life of the contract. Schedule alteration may be obtained through written request submitted 7days prior to the desired schedule alteration. Any failure to deliver service as contracted resulting in the VA obtaining alternate service or incurring damages shall be the contractor’s sole responsibility and will require full restitution of losses suffered by the VA.
• Ensure that all safety measures are followed in accordance with local and state safety laws. Report all spills, safety violations, and near misses to the COR promptly and in accordance with this contract and VA requirements.
OTHER CONSIDERATIONS
• The contractor shall be responsible for the verification of all requirements for this project.
All measurements and conditions must be verified by the contractor. Any discrepancies between field conditions, project documents, and this SOW shall be immediately brought to the COR and Contracting Officer’s attention.

• The contactor must be qualified/certified in accordance with all manufacturer requirements/local codes/regulation.
• The contractor will be required to submit a safety plan for VA review and approval prior to the performing work.
• It is the contractor’s responsibility to provide adequate documentation verifying that any equipment and/or materials proposed will meet or exceed all federal government requirements.
• Majority of the work shall be performed during normal working hours unless other wised noted by the COR. Normal working hours are 8:00 am – 4:00 pm, Monday – Friday.
• The contractor’s supervisor shall always be available to address any needs VA COR/Alternative COR may have during the period of performance, which is 60 days from start to finish and alert the Alternative COR of daily progress.
• The contractor shall thoroughly clean inside and outside of project site daily with contractor’s own equipment, including vacuums, mops, buckets, wash rags, dust mops, etc.
• The contractor shall be responsible for the removal of project demo materials/debris /equipment from this facility when complete.
CODES AND STANDARDS:
The project shall be in accordance with the latest International Building Code (IBC), National Fire Protection Associate (NFPA) standard, Uniform Federal Accessibility Standard (UFAS), VA Specifications and Standards, and local facility requirements. In the event there is a conflict between any of the standards/codes, the more stringent standard/code shall apply.
The contractor shall provide all applicable safety signage, and barricades applicable to OSHA, VHA and DOD regulations. Contractor shall comply with VHA DIRECTIVE 2004-012 and all VA Center Policy Memorandums, including but not limited to the following:
(a) Protection of patients, visitors, and employees from traumatic injury, as well as occupational and facility–associated infections.
(b) Compliance with OSHA and State construction safety regulations. Construction contracts require that the general contractor’s on-site “competent person” have completed the OSHA 30-hour construction worker course and that all other general and sub-contractor’s workers have completed the OSHA 10-hour construction worker course or other relevant competency training. The determination for training is based on the project hazards and complexity, State and Federal regulations and VA requirements. Verification of completion of training will be required prior to performing work.
(c) Compliance with EPA and State environmental regulations.
SAFETY PLAN REQUIREMENTS:
Prior to performing any work, the contractor will be required to submit a safety plan in accordance with OSHA and VA Regulations.
The safety plan shall include, but not limited to the following:
A. Safety and Health Policy (i.e., company policy including but not limited to: PPE, LOTO [Lock Out Tag Out, fall protection, etc.)
B. Safety and Health Objectives (i.e., company goals, procedures, etc.)
C. Job Site Inspections (i.e., safety inspection/potential hazards/prevention, etc.)
D. Contractor Safety Administrator (i.e., competent person/safety officer) E. Personal Protective Equipment (i.e., head, hearing, and eye protection, footwear, clothing, gloves, etc.)
F. Safety Rules (i.e., rules applicable to the work/task being performed, equipment, ladders, confined spaces, open flame, etc.)
G. Job Safety Training (i.e., OSHA, safety meetings, etc.)
• 30 Hour OSHA Construction Safety Course – competent person
• 10 Hour OSHA Construction Safety Course – all others
H. Safety Discipline (i.e., correction of violations, etc.)
I. Emergency Procedures (i.e., responsibilities, procedures, etc.)
J. Written Hazard Communication Program (i.e., responsibilities, procedures, list of hazards, MSDS, etc.)
SAFETY INSPECTIONS:
On a weekly basis, the VA will conduct a safety inspection of the work area. This inspection includes, but is not limited to the following:
A. Means of Egress
• Fire/Smoke Doors unlocked, free of obstructions.
• Means of Egress (including Exits) free of obstructions and clearly marked.
B. Fire Alarm, Fire Detection, and Fire Suppression Systems.
• Fire Alarm Pull Stations are unobstructed.
• Smoke Detector smoke caps in use or removed at end of the day.
• Sprinkler Heads are unobstructed.
C. Housekeeping and Traffic Control
• Trash and debris are removed promptly and transported outside the construction area.
• Floor mats and dust tack mats are located at the entrance to construction area.
• Tools and equipment are properly stored.
• Signs installed to restrict patient access (doors locked as needed)
• Dust control measures are in place.
• Barriers are in place and properly sealed.
D. Hazardous Chemicals/Conditions
• Appropriate storage of hazardous chemicals
• MSDS sheets are readily available on-site.
• Lock out/tag out procedures are in place.
E. Clothing
• PPE is available and properly worn.
• Proper lift, scaffolding and tie offs safety measure being used and in good conditions.
• Clothing is relatively dust free with the construction area and patient areas.
WORKMANSHIP:
All work under this contract shall conform to the standards of these specifications. The Contractor at no additional cost to the Government shall correct work not meeting these specifications. Work shall be accomplished by mechanics skilled in the trade.
WORKING CONDITIONS:
The buildings and the surrounding site areas will be occupied. These operations will continue as normal, temporary, or restricted basis for the duration of the contract. The Contractor shall take all precautions to ensure that his operations are conducted in a manner so as not to interfere with the normal operation of surrounding facilities. The Contractor shall execute work in a safe manner and shall not subject personnel to unsafe conditions. The contractor will be required to be mindful of the area operations, and patient areas while conducting the cleaning.
The Contractor shall provide safety barricades, devices, and equipment to protect personnel and property, and repair damage caused by construction operations. Take all precautions to protect the building and its occupants during the period.
SITE VISIT:
The Contractor shall visit the site to determine the full extent of the work. Failure to inspect the site will not constitute grounds for a claim after contract award.
DISPOSAL:
Disposal of materials removed under this contract at an approved disposal site in accordance with all local, state, and federal laws, regulations, and guidelines are the Contractor’s sole responsibility. The contractor shall adhere to all OSHA requirements concerning removal, transporting, and disposal of the material.
CLEAN-UP:
The Contractor shall keep the area neat and tidy and free of debris at the end of each workday.
PERMITS:
The Contractor shall be responsible for obtaining any licenses and permits and complying with any laws, codes, and regulations applicable to the execution of this work as required Federally and by the State of Texas.
WARRANTY:
The contractor shall warranty work to be free of defects and workmanship for the duration of the manufacture warranty applies. All other work shall be free of defects for the duration of 1 year.

(d) Comptroller General Examination of Record. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. SERVICE-DISABLED VETERAN-OWNED SMALL BUSINESSES (JAN 2023)
(DEVIATION)
(a) Definition. for the Department of Veterans Affairs, ‘‘Service-disabled Veteran-owned small business concern or SDVOSB’’:
(1) Means a small business concern—
(i) Not less than 51 percent of which is owned by one or more service-disabled Veterans or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more service-disabled Veterans or eligible surviving spouses (see VAAR 802.201, Surviving Spouse definition);
(ii) The management and daily business operations of which are controlled by one or more service-disabled Veterans (or eligible surviving spouses) or, in the case of a service-disabled Veteran with permanent and severe disability, the spouse or permanent caregiver of such Veteran;
(iii) The business meets Federal small business size standards for the applicable North American Industry Classification System (NAICS) code identified in the solicitation document;
(iv) The business has been certified for ownership and control pursuant to 38 U.S.C. 8127, 13 CFR 128, and is listed as certified in the SBA certification database at https://veterans.certify.sba.gov/; and
(v) The business agrees to comply with VAAR subpart 819.70 and Small Business Administration (SBA) regulations regarding small business size, government contracting, and the Veteran Small Business Certification Program at 13 CFR parts 121, 125, and 128.

When awarded a contract action, including orders under multipleaward contracts, an SDVOSB agrees that in the performance of the contract, the SDVOSB shall comply with requirements in VAAR subpart 819.70 and SBA regulations on small business size, and government contracting programs at 13 CFR part 121 and part 125, including the non-manufacturer rule and limitations on subcontracting (LOS) requirements in 13 CFR 121.406(b) and 13 CFR 125.6. For the purpose of limitations on subcontracting, only certified SDVOSBs listed in the SBA certification database (including independent contractors) shall be considered eligible and/or ‘‘similarly situated’’ (i.e., a firm that has the same small business program status as the prime contractor). An otherwise eligible firm further agrees to comply with the required LOS certification requirements in this solicitation (see 852.219–75 or 852.219–76 as applicable). These requirements are summarized as follows:
(1) Services. In the case of a contract for services (except construction), the SDVOSB prime contractor will not pay more than 50% of the amount paid by the government to the prime for contract performance to firms that are not certified SDVOSBs listed in the SBA certification database (excluding direct costs to the extent they are not the principal purpose of the acquisition and the SDVOSB/ VOSB does not provide the service, such as airline travel, cloud computing services, or mass media purchases). When a contract includes both services and supplies, the 50 percent limitation shall apply only to the service portion of the contract.
(2) Supplies/products.
(i) In the case of a contract for supplies or products (other than from a non-manufacturer of such supplies), the SDVOSB prime contractor will not pay more than 50% of the amount paid by the government to the prime for contract performance, excluding the cost of materials, to firms that are not certified SDVOSBs listed in the SBA certification database. When a contract includes both supply and services, the 50 percent limitation shall apply only to the supply portion of the contract.
(ii) In the case of a contract for supplies from a non-manufacturer, the SDVOSB prime contractor will supply the product of a domestic small business manufacturer or processor, unless a waiver as described in 13 CFR 121.406(b)(5) has been granted. Refer to 13 CFR 125.6(a)(2)(ii) for guidance pertaining to multiple item procurements.
(3) General construction. In the case of a contract for general construction, the SDVOSB prime contractor will not pay more than 85% of the amount paid by the government to the prime for contract performance, excluding the cost of materials, to firms that are not certified SDVOSBs listed in the SBA certification database.
(4) Special trade construction contractors. In the case of a contract for special trade contractors, no more than 75% of the amount paid by the government to the prime for contract performance, excluding the cost of materials, may be paid to firms that are not certified SDVOSBs listed in the SBA certification database.
(5) Subcontracting. An SDVOSB subcontractor must meet the NAICS size standard assigned by the prime contractor and be certified and listed in the SBA certification database to count as similarly situated. Any work that a first tier SDVOSB subcontractor further subcontracts will count towards the percent of subcontract amount that cannot be exceeded. For supply or construction contracts, the cost of materials is excluded and not considered to be subcontracted. When a contract includes both services and supplies, the 50 percent limitation shall apply only to the portion of the contract with the preponderance of the expenditure upon which the assigned NAICS is based. For information and more specific requirements, refer to 13
CFR 125.6.
(e) Required limitations on subcontracting compliance measurement period. An SDVOSB shall comply with the limitations on subcontracting as follows:
[] By the end of the base term of the contract or order, and then by the end of each subsequent option period; or
[X] By the end of the performance period for each order issued under the contract.
(f) Joint ventures. A joint venture may be considered eligible as an SDVOSB if the joint venture complies with the requirements in 13 CFR 128.402 and the managing joint venture partner makes the representations under paragraph (c) of this clause. A joint venture agrees that, in the performance of the contract, the applicable percentage specified in paragraph (d) of this clause will be performed by the aggregate of the joint venture participants.
(g) Precedence. The VA Veterans First Contracting Program, as defined in VAAR 802.101, subpart 819.70, and this clause, takes precedence over any inconsistencies between the requirements of the SBA Veteran Small Business Certification Program and the VA Veterans First Contracting Program.
(h) Misrepresentation. Pursuant to 38 U.S.C. 8127(g), any business concern, including all its principals, that is determined by VA to have willfully and intentionally misrepresented a company’s SDVOSB status is subject to debarment from contracting with the Department for a period of not less than five years (see VAAR 809.406–2 Causes for Debarment).
