QUESTIONS AND ANSWERS
1. Please provide the current contract number for the Mobile MRI services being provided.
A.1 Contract 36C25719C0239
2. Please provide the age/type of unit currently provided on the contract for Mobile MRI services.
A.2 The current MRI machine is a Philips unit. This unit was <5 years old at the beginning of the contract.
3. What is the transition plan between the incumbent system and the new system for a POP date of 9/1/25.
A.3 Plan – remove current unit and plan for new install/setup on 9/1 with ~2-3 days for setup and testing then go live.
4. Please provide blueprint/specs on the location of the mobile unit and all utility connections (power/water/etc).
A.4 Blueprints/Specs would need to be provided from facilities however, this would be replacing an existing MRI unit located in the motorcycle parking lot outside of the Teague Tower at the Temple VA.
5. Page 13, 9c states: “Contractor shall be responsible for the transportation of the Mobile MRI coach to and from CTVHCS”
a. Please confirm that this system will be stationary at the VA Temple facility and not moved on a daily/weekly basis until the contract expires.
A.5 Contractor will only be responsible for the transportation of the unit to and from the Temple VA at the beginning and ending of the contract period. The unit will not move, while in use by the VA, for the duration of the contract.
6. PWS 3. CONTRACTOR RESPONSIBILITIES states “The operating system must be Windows 10 or newer/ Lynux”, however, Section Page 71, Part 1, section states, “The mobile 1.5T MRI unit uses an Operating system must be Windows 7 or newer”. Per current Federal Regulations, Windows 10 is required on all equipment.
a. Can the government please confirm Windows 10 requirement and amend the scope of work accordingly.
A.6 Yes the system should be completely windows 10 or newer.
7. Factor 2 Past Performance States “Offerors may be asked to clarify certain aspects of their offers or respond to adverse past performance information to which the offeror has not previously had an opportunity to respond. Adverse past performance is defined as past performance information that supports a less than satisfactory rating on any evaluation element or any unfavorable comments received from sources without a formal rating system.”
a. Can the government please provide the Past Performance Risk Rating and Confidence Assessments that will be used by the Past Performance Evaluation Team.
A.7 FACTOR 2 - PAST PERFORMANCE: Offerors shall submit two (2) current or recently completed from Federal, State, local government, or private contracts under which you have provided the same or similar services required in this solicitation within the last three (3) years. Provide contact name, date of performance, and point of contact, telephone number, and e-mail address. Past performance will be reviewed to assure that the Contractor has relevant and successful performance history.
The Government may also consider other relevant information when evaluating an offeror’s past/present performance obtained from other federal agency web sites, i.e., the Contractor Performance Assessment Reporting System (CPARS). Past Performance Ratings: Exceptional / Very Good / Satisfactory / Marginal / Unsatisfactory / Neutral.
8. Will the government review CPARS Integrity Reports for all responding vendors relevant to documented Terminations for Cause, Default, or Material Failure to Comply, as part of its Past Performance review.
A.8 Yes.
ADDENDUM to FAR 52.212-1 INSTRUCTIONS TO OFFERORS—COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES Provisions that are incorporated by reference (by Citation Number, Title, and Date), have the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available.
The following provisions are incorporated into 52.212-1 as an addendum to this solicitation:
A. GENERAL.
1. The following instructions are provided to ensure the submission of information necessary to properly evaluate each Offeror’s quote. Submit complete quotes as detailed in this section and FAR 52.212-2, Evaluation—Commercial Items (NOV 2021). Include only information relevant to this solicitation. If the solicitation is amended, the Offeror is responsible to acknowledge receipt of the amendment prior to the hour and date specified in the amendment. Failure to acknowledge receipt of the amendment may result in rejection of your quote. Failure to properly provide all information required by this solicitation may result in an invalid quote. Read entire solicitation and follow instructions accordingly because lack of required information and/or documents could result in a determination that the quote is non-responsive.
Only VA verified SDVOSBs in the Veteran Small Business Certification (VetCert) https://veterans.certify.sba.gov/) at the time of submission of offer will be considered for award and non-VetCert verified firms will be considered non-responsive and ineligible for award. The North American Industry Classification System (NAICS) Code 621512 applies to this acquisition. Offerors shall be registered as a Small Business for NAICS 621512 and shall be represented as such within its System for Award Management (SAM) registration. To be considered for award a SDVOSB Offeror shall be verified in VetCert as an SDVOSB and shall be registered as a small business for NAICS 621512 in SAM.
2. Inquiry or questions: All questions regarding this solicitation must be submitted and received by 03:00 PM EDT on Monday, 30 June 2025 via email to the Contract Specialist Brandy Riha at brandy.riha@va.gov and the Contracting Officer Elsa Gomez at elsa.gomez@va.gov. Please reference the following in the subject line of the email: 36C25725Q0664 – Mobile MRI and Technologist Services. All questions regarding this solicitation must be submitted via email. Answers to questions will be provided VIA an Amendment.
3. Submission deadline: Offers are due on Monday, 21 July 2025 at 03:00 PM EDT. Submit your offer by responding to this RFQ via email to Contract Specialist Brandy Riha at brandy.riha@va.gov and the Contracting Officer Elsa Gomez at elsa.gomez@va.gov. All attachments in response to this RFQ shall be either Adobe or Microsoft Office Word Format. It is the offeror’s responsibility to ensure all required documents are included and completed as required by this solicitation. Offeror(s) shall be considered non-responsive if all requested information is not received by the specified due date.
4. Requirements for Submittal: The following sub-sections are the requirements to be submitted by the offeror. Late offers will not be accepted. Offerors are advised that large attachments to emails may delay transmission and should plan accordingly. Offerors may want to request acknowledgement from the Government POC of its offer. Offerors who fail to request confirmation that their quotes were received, may not be considered for award. Limit email sizes to 15MB.

B. FORMAT.
1. To assure the timely and equitable evaluation of quotes, the Offeror must follow the instructions contained herein. The quote must be complete and respond directly to the requirements of the solicitation. The response shall consist of a total of one (1) quote copy.

6. VAAR 852.219-75 VA Notice of Limitations on Subcontracting—Certificate of Compliance for Services and Construction (JAN 2023) (DEVIATION)
6. FAR 52.212-3 Offeror Representations and Certifications – Commercial Items (MAY 2024), only paragraph (b) of that provision must be returned if the Offeror has completed the annual representations (reps) and certifications (certs) electronically at http://www.sam.gov. If the Offeror has not completed the annual online reps and certs, then the Offeror shall complete paragraphs (c) through (u) of this provision. The Offeror shall provide either the information in paragraph (b) or the information contained in paragraphs (c) through (u) with its quote.
1. Technical Capabilities. The offeror shall provide documentation that demonstrates that the Offeror can provide a mobile 1.5T MRI unit (5 years old or newer) (including scanner, acquisition and processing station, and physician viewing stations) with a high-performance gradient system for use at CTVHCS.
The documentation must demonstrate:
(1) The operating system must be Windows 10 or newer/ Lynux
(2) MRI system minimum requirements:
a. 45 mT/m gradient strength “minimum prerequisites”
b. 200 T/m/s Slew rate [T/m/s} “minimum prerequisites”
c. 70 cm Bore dimensions.
d. 550 lbs or greater scanner table weight limit
e. Minimum number of receiver channels – 45
f. Motorized table with integrated coil design and ports
g. Patient positioning and support aids
h. MRI compatible dual head injector pedestal or ceiling mounted.
i. Zero boil-off cryogen technology
j. Vendors are expected to provide and install all cryogens.
k. Chiller must meet or exceed MRI and location needs.
i. Central Texas temperature can exceed 100 deg F
l. All phantoms that are required for calibration and performance verification.
i. Vendor must provide ACR compliant phantom for system QA.
(3) Software minimum requirements:
a. Motion correction technology
b. Magnetic Artifact Reduction
c. Noise reduction
d. ECG (VCG), respiratory, and peripheral pulse gating/triggering
e. Real-time MIP, MPR, and 3D surface rendering
f. Advanced non-contrast MRA imaging technology

(8) The scanner must include some form of effective noise abatement system for the patients.
(9) The scanner must offer a patient entertainment system that enables the patient to experience digital media during the MRI scan procedures.
The Offeror shall provide a copy of a current, comprehensive MRI Safety Program policy to which each technologist is expected to comply. MRI Technologist is required to complete CTVHCS MRI Safety Training yearly.
The Offeror's Technologist shall possess a current Radiologic Technologist (RT) license. A copy of all applicable operators’ license(s) shall be provided with the proposal along with a copy of the updated license immediately upon renewal.
Evidence of the Offeror’s Technologist’s competency review shall be provided with the proposal.
The Offeror shall be required to maintain documentation and provide copies, with their proposal, of the following for each employee working under the contract:

| (2) | a current competency assessment checklist (an assessment of knowledge, skills, abilities and behaviors required to perform a job correctly and skillfully; includes age-specific knowledge and skills required to provide care for certain patient populations, as appropriate.) | ||
| (3) | a listing of relevant continuing education for the last two years. | ||
| (4) | health examination records of all individuals performing work under this contract to include: | ||
| (a) | Annual TB Skin Test and recent (within the last year) chest X-ray if there is a history of positive TB skin test |
(b) Evidence of Hepatitis B immunity (hepatitis immune titer, if provider has had the series of shots; if no immunity, evidence that provider has started the Hepatitis B vaccination series.)
(c) Varicella titer if provider has not had chicken pox and has direct patient contact.
The Offeror shall provide, with their proposal, a copy of their Quality Assurance Program Plan.
Sign and acknowledge (either manually or electronically) understanding of and responsibility for compliance with the Contractor Rules of Behavior, attached, relating to access to VA information and information systems.
Do not include Social Security Numbers on any documentation presented in the RFQ unless contacted and requested telephonically by the Contract Specialist.
a. Part II:
PAST PERFORMANCE. Contractors shall submit two (2) current or recently completed from Federal, State, local government, or private contracts under which you have provided the same or similar services required in this solicitation within the last three (3) years. Provide contact name, date of performance, and point of contact, telephone number, and e-mail address. Past performance will be reviewed to assure that the Contractor has relevant and successful performance history.
b. Part III:
PRICE. Offerors shall submit their Price using B.2 Schedule of Services. Insert proposed pricing for the firm-fixed price Line-Item Numbers (LINs) 0001 through 4001. Additional sheets may be attached if required. Each quote must include an offer to provide all basic services.
Note 1: It is a violation of the Federal Acquisition Regulation for vendors and Government employees to discuss the solicitation prior to award. All communications are required to take place between interested offerors and the Contracting Officer as listed in block 7a of the SF 1449.
Note 2: Offerors are advised that any communication with employees of the affected VA Medical Health Care System regarding this competitive solicitation may result in a determination by the Contracting Officer that they are no longer eligible to be considered for award.
Note 3: A single award will be made from this solicitation.
Note 4: This will be a total SDVOSB set-aside.

E.9 52.212-2 EVALUATION—COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES (NOV 2021)
(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered.

Technical and past performance, when combined are significantly more important when compared to price.
(b) Evaluation Approach. The Government will evaluate quotations using the comparative evaluation process outlined in FAR 13.106-2 (b) (3), where quotations will be compared to one another to determine which provides the best benefit to the Government. The Government reserves the right to consider a quotation other than the lowest price that provides additional benefit(s). Quotations may exceed minimum requirements of the solicitation. The Government reserves the right to select a quotation that provides benefit to the Government that exceeds the minimum requirements of the solicitation but is not required to do so. Each response must meet the minimum requirements of the solicitation. The evaluation will consider the following:
FACTOR 1 – TECHNICAL CAPABILITIES
Evaluation Criteria: The offeror will be evaluated to the extent to which it can meet and/or exceed the Government’s requirements as outlined in this solicitation and based on the information requested in the instructions to offerors section of the solicitation.
Technical Capability. Offeror must meet all technical specifications identified within this solicitation. Particular attention must be given to the Performance Work Statement, Paragraph 3 – Contractor Responsibilities, and Paragraph 6 – Contractor Personnel Qualifications/Policies. “Technical Capability” will be rated as a “Acceptable or Unacceptable.”

1. The system must be capable of performing 2D, 3D TOF & PCA, and total Body contrast enhanced MRA (intracranial and body) studies.
1. The scanner must include some form of effective noise abatement system for the patients.
8. The MRI unit must offer a patient entertainment system that enables the patient to experience digital media during the MRI scan procedures.
The Government will verify the offeror has a current, comprehensive MRI Safety Program policy to which each technologist is expected to comply. MRI Technologist is required to complete CTVHCS MRI Safety Training yearly.
The Government will verify the technologist possesses a current Radiologic Technologist (RT) license.
The Government will verify evidence of the offeror’s Technologist’s competency review shall be provided with the quote.
The Government will verify documentation and copies of the offeror of the following for each employee working under the contract:
| (1) | credentials and qualifications for the job |
| (2) | a current competency assessment checklist (an assessment of knowledge, skills, abilities and behaviors required to perform a job correctly and skillfully; includes age-specific knowledge and skills required to provide care for certain patient populations, as appropriate.) |
| (3) | a listing of relevant continuing education for the last two years. |
| (4) | health examination records of all individuals performing work under this contract to include: |
| (a) | Annual TB Skin Test and recent (within the last year) chest X-ray if there is a history of positive TB skin test |
| (b) | Evidence of Hepatitis B immunity (hepatitis immune titer, if provider has had the series of shots; if no immunity, evidence that provider has started the Hepatitis B vaccination series.) |
| (c) | Varicella titer if provider has not had chicken pox and has direct patient contact. |
The Government will verify a copy of the offeror’s Quality Assurance Program Plan.
The Government will verify that the offeror submits a signed copy (either manually or electronically) acknowledging and understanding of and responsibility for compliance with the Contractor Rules of Behavior, attached, relating to access to VA information and information systems.

FACTOR 2 - PAST PERFORMANCE
Evaluation Criteria: The past performance evaluation will assess the relative risks associated with a quoter’s likelihood of success in fulfilling the solicitation’s requirements as indicated by the quoter’s record of past performance. Offerors shall submit two (2) current or recently completed from Federal, State, local government, or private contracts under which you have provided the same or similar services required in this solicitation within the last three (3) years. Provide contact name, date of performance, and point of contact, telephone number, and e-mail address. Past performance will be reviewed to assure that the Contractor has relevant and successful performance history.
Other Past Performance Considerations: The Government may also consider other relevant information when evaluating an offeror’s past/present performance obtained from other federal agency web sites, i.e., the Contractor Performance Assessment Reporting System (CPARS).
Offerors may be asked to clarify certain aspects of their offers or respond to adverse past performance information to which the offeror has not previously had an opportunity to respond. Adverse past performance is defined as past performance information that supports a less than satisfactory rating on any evaluation element or any unfavorable comments received from sources without a formal rating system.
The Government reserves the right to consider past performance information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement provided by the offeror, as well as information obtained from any other sources, to establish a past performance rating.
FACTOR - PRICE
Offerors shall submit their Price using B.2 Schedule of Services. Additional sheets may be attached if required. Each quote must include an offer to provide all basic services. The Government will evaluate the price by adding the total of all line-item prices. The Total Evaluated Price will be that sum. Price shall be evaluated for fairness and reasonableness and will be compared against competing quotes and the Independent Government Cost Estimate (IGCE).
(c) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). As part of price evaluation, the Government will evaluate its option to extend services (see FAR Clause 52.217-8). The evaluation will assume that the prices for any option exercised under FAR 52.217-8 will be at the same rates as those in effect under the contract at the time the option is exercised. The evaluation will therefore assume that the addition of the price or prices of any possible extension or extensions under FAR 52.217-8 to the total price for the basic requirement and the total price for the priced options has the same effect on the total price of all proposals relative to each other, and will not affect the ranking of proposals based on price, unless, after reviewing the proposals, the Government determines that there is a basis for finding otherwise. This evaluation will not obligate the Government to exercise any option under FAR 52.217-8.
(d) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
