About this file

This is a Performance Relevancy Survey form (Exhibit A) for the federal contract opportunity to Renovate Radiation Oncology Department (Project No. 586-26-100, Solicitation 36C25626R0072).

Part 1 requires offerors to submit information on no more than three past performance projects completed within five years of the solicitation issue date. For each project, offerors must provide the project/contract title, contract number, location, original and current contract values, start and end dates, on-time completion status, any liquidated damages assessed, and CPARS availability. Offerors must classify each project as Very Relevant (essentially the same scope, magnitude, and complexities), Relevant (similar scope, magnitude, and complexities), or Somewhat Relevant (some of the scope and magnitude required), and provide detailed descriptions of work performed including subcontractor involvement. Offerors must also identify a point of contact from the reference organization who will receive the questionnaire/survey, including their name, title, agency, email, and phone number. Offerors are responsible for ensuring references submit completed questionnaires prior to solicitation close.

Part 2 requests company-wide information provided only once in the submission package, including assessment of liquidated damages on contracts within the past five years, discussion of problems encountered on relevant projects with corrective actions taken, explanation of any marginal CPARS ratings, professional/industry awards or letters of recognition from the past five years, unique skills and accomplishments with supporting documentation, and technical areas uniquely relevant to the radiation oncology renovation project, specifically identifying work elements performed by the prime firm and major subcontractors.
