About this file
This document is a master table of proposal requirements for a two-phase competitive solicitation process.
Phase I is a technical qualifications round with submissions limited to one PDF file not exceeding 30 pages (excluding title page and table of contents). Proposals must include a preface identifying the offeror/joint venture structure, confirming Service-Disabled Veteran-Owned Small Business (SDVOSB) eligibility, designating a main point of contact, and summarizing team composition. Technical content addresses three factors: (1) Technical Approach narrative covering design, construction methodology, quality control, phasing, service continuity, and risk mitigation (maximum 3 pages); (2) Technical Qualifications consisting of resumes for key architecture/engineering and construction personnel plus identified subcontractor leads at 2 pages per resume; and (3) Past Performance documenting up to three completed projects using one page per project with supporting Exhibits A and B as applicable. Subcontractor past performance documentation requires Exhibit B consent forms. Supporting CPARS and PPQ documents are appendices not counted toward page limits. Submissions must be searchable PDFs formatted 8.5x11 inches (11x17 allowed for schedules) with company name and page numbers, delivered via single email with file size not exceeding 5MB.
Phase II is an invitation-only round limited to the four most highly qualified Phase I offerors, requiring two separately evaluated volumes each not exceeding 30 pages. Volume I (Technical, Non-Price) contains a 2-page design concept schematic addressing equipment layout, clearances, mechanical spaces, laydown area, site plan, and phasing; a 2-page draft schedule using fictitious notice-to-proceed of December 1, 2026, showing design milestones, HEFP reviews, construction sequencing, and concurrency; and a 1-page schedule narrative addressing risk mitigation, occupied medical environment constraints, and sequencing rationale. Volume II (Administrative & Price) includes SF 1442 form with price schedule, representations and certifications acknowledging amendments and committing to firm fixed price with no exceptions or conditions; a commercial bid bond per FAR 52.228-1 (commercial bonds not acceptable); full cost breakdown aligned to the price schedule; and Exhibit C documenting OSHA/EPA certifications, EMR documentation on carrier letterhead, NCCI/state ratings if needed, and Unity Rating if no claims history, with missing documentation potentially resulting in non-responsibility determination. Phase II submissions require two separate emails (one per volume) with each PDF searchable, 5MB or less, and labeled with company name, address, solicitation number, and page numbers.
