Radiation Shielding Design Report Summary

This document is a Radiation Shielding Design report prepared by Olympic Health Physics for VA St. Louis Health Care System's Computed Tomography Room (A259). The design was completed on May 3, 2026, by Medical Physicist Eric Hooper, MS, CHP, DABSNM, and conforms to NCRP Report No. 147 standards for structural shielding design in medical X-ray imaging facilities. The facility operates a Siemens Naeotom Alpha CT scanner with an operational workload of 100 head CT scans and 150 body CT scans per week at a maximum of 140 kVp, with 40% of scans utilizing contrast. Design goals specify 0.1 mGy/week for controlled areas and 0.02 mGy/week for uncontrolled areas.

The shielding design prescribes minimum barrier installations across multiple barriers including walls, doors, floors, and ceilings. Key shielding requirements include 1/32" lead on South Wall A and Room Door A (hallway barriers), 1/16" lead on West Wall B1 (adjacent control), 1/32" lead on East Wall D and Storage Door D (adjacent control), 1/16" lead on Control Door E and Control Wall E, and 1/32" lead on both floor and ceiling to protect occupied spaces above and below. The North Wall C requires no additional shielding. All walls requiring lead shielding must extend from floor to a minimum height of 7 feet, with special attention to door frames, window frames, and penetrations for electrical and HVAC systems to maintain barrier integrity. The report includes detailed installation specifications, barrier calculations, floor maps, supplemental design guidance, and example shielding details for doors, windows, and penetrations. A Shielding Verification Survey is strongly recommended after barrier installation to validate proper placement and thickness.

The assumptions used in this report were supplied by the facility. If the actual CT usage is different than that provided by the facility or if there are increases to the workload or modifications made to the barriers, additional shielding may be required. In such cases, there should be consultation with a medical physicist. A copy of this report should be maintained by the facility for the life of this room.

Shielding is an integral part of rooms utilizing ionizing radiation and is required to protect staff, patients, and members of the public in adjacent spaces. The contents of this report should be reviewed carefully to ensure adjacent spaces are adequately protected.

Barrier penetrations, such as those for outlets, switches, light boxes, etc, should be wrapped or backed with equivalent shielding as specified for the prescribed wall. Special attention should be given to penetrations and joints as this is often a point of failure for shielded spaces.

Walls requiring lead will only need shielding installed on one side of the wall. For example, lead-lined drywall may be installed on one side of the studs while regular GWB may be installed on the other side.

All walls requiring shielding must have the shielding extend from the floor to a height of at least 7 feet.

Shielding Calculations

1 Calculations were made to both the storage room and the control space beyond. The limiting calculations were due to the control room and are presented in the table above.

2 Calculations were made to occupied spaces above on the periphery of the scan room. Due to the increased distance, the vertical barriers only need to extend to a height of 7 feet.

Shielding Verification Surveys It is strongly recommended by NCRP Report. No. 147, and a requirement of many State Regulatory Agencies, that a Shielding Verification Survey be performed after the installation of protective barriers. These verification surveys evaluate the proper placement and thickness of prescribed barriers as well as inspects for any voids, gaps, or penetrations. For assistance with a Shielding Verification Survey, please each out to Olympic Health Physics.

3. The Viewing Window should allow for continuous viewing of the patient and be at least 1 sq. ft.

Supplemental Design and Building Information

Background The information provided in this section is designed to assist the facility, architect, and/or builder in successful installation of the required shielding for this space. This section is INFORMATIONAL ONLY and is to be used at the discretion of the facility. Olympic Health Physics makes no guarantees or warranties of any kind, express or implied, or assumes liability for the installation of protective barriers.

Regulatory Many states have regulations that govern the oversight of protective barriers to ensure barriers are adequate.

Regulations typically follow accepted published guidance documents, such as NCRP Report No. 147. In addition, some states have a formal review process that requires submittal of a Shielding Design by a Qualified Expert before construction. It is recommended to be familiar with the regulations specific to your State and installation to ensure the facility adheres to all relevant regulations.

Postings & Signage Radiation Area means any area, accessible to personnel, in which there exists radiation at such levels that a major portion of the body could receive in any 1 hour a dose in excess of 5 millirem. As such, it is recommended that this room be posted with a "Caution, Radiation Area" sign at each entrance. A lighted sign outside the room indicating when the beam is energized may also be installed, but is not required.

Control Booth There are several design elements that are specific to Operator Control Booth's.

1. The operator must be allotted at least 7.5 sq. ft. of unobstructed floor space in the Control Booth. No one dimension should be less than 2 feet.

2. The X-Ray Control Switch must be positioned and permanently mounted at least 40 inches from the outside edge of the Control Booth. The position of the X-Ray Control Switch should allow for continuous viewing of the patient during exposure.

Barrier Installation

Walls Sheet lead should be affixed to or covered with wallboard, plywood, or other rigid material. Where the edges of two lead sheets meet, continuity shall be ensured at the joints by sufficiently overlapping the lead sheets not less than 1/2 inch. In cases where a butt joint is used, a batten of the same lead thickness as the wall may be used to ensure adequate coverage of the gap.

Lead should be affixed to the wall using steel nails or screws. The penetrations created by screws or nails need not be covered with lead discs or supplementary lead.

All walls requiring shielding must have the shielding extend from the floor to a height of at least 7 feet, unless otherwise stated.

Doors for shielding applications are most commonly constructed of Wood or Steel. Wood is a poor attenuator of scatter radiation and is typically reserved only for low energy applications. The most commonly used shielded door is a Wood Door that has embedded with Lead. In some situations, a Steel Door may be substituted for a Wood/Lead Door, but requires a more thorough calculation to determine the appropriate Steel thickness.

Doors, door frames, and jambs shall maintain the same shielding integrity as specified for the barrier.

Junctions between door frames and adjoining walls should be constructed to create a 1 cm overlap between the frame and the wall. Care should be taken to ensure the jamb and wall has adequate overlap as this can be a common failure point found during Shielding Verification Surveys.

Doors should be installed so that the bottom edge comes as close as possible to the finished floor. There should be no more than a 1 cm gap between the bottom of the door and the finished floor. If the gap is greater than 1/2 inch, a shielded threshold should be installed.

If a Viewing Window is placed in a door, the window must have the same lead thickness equivalency as prescribed in the door.

Windows Windows and window frames shall maintain the same shielding integrity as specified for the barrier.

Junctions between window frames and adjoining walls should be constructed to create a 1 cm overlap between the frame and the wall. Care should be taken to ensure the frame and wall has adequate overlap as this can be a common failure point found during Shielding Verification Surveys.

As described in the Control Booth section above, any window located in the Control Booth should be at least 1 sq. foot and allow for continuous viewing of the patient during an exposure.

Penetrations All barrier penetrations such as those made for pipes, air ducts, conduits, electrical boxes, switches, etc., require the use of either lead baffles or lead backing with sufficient overlap in order to maintain the shielding integrity of the wall. The lead backing should be of at least the same thickness as prescribed for the wall in which the penetration is made.

Barrier penetrations can successfully be shielded using commercially available, pre-made, lead lined boxes.

Alternatively, sheet lead can be used to back and/or wrap barrier penetrations. Care should be taken to ensure adequate overlap of the penetration to maintain the barrier integrity.
