About this file
This is a Confined Space Entry Procedure document detailing the required safety protocols for entering sewer manholes. The procedure outlines specific steps including placing orange cones around the work area, ventilating the space for one hour, obtaining proper permits, and assigning distinct roles for Entry Supervisor and Entry Attendant.
The document requires gas monitoring before entry, use of safety harness by the Authorized Entrant, setup of an extrication tripod, and continuous monitoring by the Entry Attendant. It includes emergency procedures, prohibiting rescue attempts by the Entry Attendant if the Authorized Entrant cannot exit, and requiring 911 calls for emergencies. The document concludes with an annual review section to ensure procedures are followed and remain adequate. This appears to be related to a Department of Veterans Affairs solicitation for confined space assessment at the Hines Campus, though the specific requirements of that solicitation are not provided in the file.
