About this file

This file is a Confined Space Entry Procedure document detailing the required safety protocols for entering a sewer ejector pit. The document outlines a comprehensive step-by-step process that includes hazard identification (inhalation, entrapment, engulfment), pre-entry requirements (ventilation, permits, lockout procedures), and safety measures during entry operations.

The procedure mandates specific roles including an Entry Supervisor, Entry Attendant, and Authorized Entrant, with explicit prohibition of the same person serving as both supervisor and attendant. Key safety requirements include: one-hour ventilation period, gas monitoring before and during entry, use of safety harness, extrication tripod setup, continuous monitoring by the Entry Attendant, and emergency procedures. The document includes an annual review section to ensure procedures remain adequate and are properly followed. This appears to be related to a Department of Veterans Affairs solicitation for confined space assessment at the Hines Campus.
